They also updated policy to ensure that interim and new directors credentials are submitted timely to allow review and approval by TDHS Licensing prior to assuming duties.

    Child care has a written supervision plan that does not include Name to face roll call. Two classrooms were observed transitioning to the playground. Both classroom educators completed name to face roll call while leaving the classroom but failed to complete name to face roll call upon arrival at the playground. 1240-04-01-.11 Supervision 4(b) Child care agencies shall develop and follow a written playground supervision plan that includes: 1. Arrival and departure procedures; 2. Playground design and placement of equipment; 3. Individual staff duties to ensure age-appropriate supervision can be given to each child at all times; 4. Individual children's needs, including high risk behaviors; 5. Emergency procedures, including communication with other staff; and 6. Roll call before leaving classroom and upon arrival at playground and prior to leaving playground and upon arrival in classroom. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: CCR&R referral was made for supervision coaching. All staff were re-trained by director on how to do name to face transitions and signed a document stating they had received training and directors are monitoring documentation of name to face throughout the day. Playground supervision plan was updated to reflect rules requirements.
