Licensing Consultant (LC) conducted an unannounced complaint visit on April 11, 2025. While reviewing the file of the child named in the complaint, LC did not observe a completed incident report regarding the matter reported. The educator reported that she informed the parent of the incident. However, she did not complete the incident report form. — Visit: Complaint Investigation,Technical Assistance — Action taken: The provider ensured that an incident report was completed and signed by the parents. The provider has committed to review with all employees when incident reports are required and the difference between an incident report and an accident report. The provider stated that they will review Comp. R.& Regs. 1240-04-01-.09 Incident reporting with staff.

1240-04-01-.10 Duty to Report Child Abuse and Neglect
Licensing Consultant (LC) conducted an unannounced complaint visit on April 11, 2025. While interviewing staff members, LC was made aware that the director was notified of the incident involving the alleged physical abuse of the child named in the complaint. The Director failed to report the incident to the LC and failed to notify the Department of Children Services (DCS). — Visit: Complaint Investigation,Technical Assistance — Action taken: The provider will ensure that Every classroom has 2 teachers at all times during the school day. The provider will continue their professional development training with intentional focus on how to handle situations when their patience is being tested by difficult situations. The provider ensures classrooms are equipped with land lines and extensions are posted above the phones. Every teacher carries a walkie talkie and can ask for help in difficult situations. Professional development Training focused on self-evaluation and the ability to recognize when teachers are feeling frustrated and learning to ask for help is another important skill to learn and practice.
