During an unannounced safety plan monitoring visit on 04/10/2025, the Licensing Consultant observed an infant in the Infant A classroom in a high chair with a bottle propped with a soft toy. The educator was at the sink area doing something else and was not supervising the infant in the high chair. During an unannounced safety plan monitoring visit on 04/10/2025, the Licensing consultant observed an infant in a high chair with a bottle propped with a soft toy. Infants shall be held while drinking from a bottle and bottles should not be propped. — Visit: Safety Plan Monitoring,Technical Assistance — Action taken: Per the agency, the educator was retrained on proper infant feeding. The infant educators were reminded (by management) that infant bottles should not be propped.

During an unannounced injury complaint visit on January 21, 2025, the Licensing Consultant checked the child's file and noticed that the incident occurred on January 16, 2025, however, the incident was reported on January 17, 2025. — Visit: Incident or Injury Investigation — Action taken: The agency states that they will make sure to report incidents the same day that the incidents happen.

During an unannounced injury complaint visit on January 21, 2025, the Licensing Consultant observed the educators in an infant class did not have documentation of infants sleeping or the times that they were placed in their cribs asleep. — Visit: Incident or Injury Investigation — Action taken: The agency's management met with Infant A teachers to discuss improving/completing documentation of infant sleep charts.

1240-04-01-.12 Health and Safety

During an announced visit on 10/21/2024, the Licensing Consultant observed four educators who have been employed over 90 days have not completed the pediatric or age appropriate first aid and CPR training. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Per management at the agency: "When hiring a new teacher, we are going to double check to make sure they are CPR certified before they start."
