   Evidence gathered including video surveillance and verbal admissions from multiple agency staff during LC Eldridge's investigation, confirmed that a serious incident occurred on May 7th, 2026. The incident involved a 3-year-old child who walked out of the agency without the knowledge of the staff and was left unattended outside of the building for at least 10 minutes. — Visit: Incident or Injury Investigation — Action taken: Jamestown Head Start staff will be provided with training on developmentally appropriate practices and classroom guidance/behavior management training, including positive guidance and redirection strategies for preschool-aged children. Staff will also participate in additional child safety and active supervision training to reinforce supervision expectations and transition procedures. In addition, the program will ensure safety gates are installed and utilized at classroom exits, and that classroom doors remain properly closed and secured to provide multiple barriers preventing children from exiting the building unsupervised. Door bell chimes will also be installed so that the sound of the chimes will alert staff any time the classroom doors are opened. The program will continue to review and monitor active supervision practices to ensure compliance with Tennessee DHS licensing requirements.
