1240-04-01-.04 Procedures for Obtaining a License

Submission of required documentation, including when an employee begins and is terminated must be documented and a change form must be provided in a timely manner. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: A change report was completed terminating previous employees. Ongoing, a change report form will be completed when employees are addded or terminated.

1240-04-01-.11 Supervision

Infant room briefly left unattended by staff but a volunteer quickly stepped in. — Visit: Complaint Investigation,Technical Assistance — Action taken: The square footage was remeasured in the infant/toddler room to get the correct number of children that the room would allow. Enrollment has been terminated for some children to maintain staff to children ratio.

1240-04-01-.06 Staff

Agency failed to provide adequate and appropriate staffing to ensure classroom child-to-adult ratios and could not produce required staffing records. — Visit: Complaint Investigation,Technical Assistance — Action taken: All Volunteers and Subs will sign in and out at front desk upon arrival for the day. All employees have been aware of the this change and will not let volunteers or subs to act as the only adult in classrooms. Volunteer, Linda Matthews will have all necessary pre employment verify to meet the stardards to be an employee of Small World Daycare.

1240-04-01-.12 Health and Safety

Agency failed to properly store "keep-out-of-reach" items, allowing access to children. — Visit: Complaint Investigation,Technical Assistance — Action taken: All Staff has been trained with how to make sure all cleaners and diapers bags to be away from children at all times. All Cleaners has been put up into a lock box where no children can get to them. All Diapers bags are put up out of reach from the children.

1240-04-01-.08 Record Keeping

Child's sign-in/out attendance sheets were incomplete. — Visit: Complaint Investigation,Technical Assistance — Action taken: Sign in Sheets are being changed to include Child Full name and signature with date and class in which child/children classrooms. New sign in sheets updated to include child full name, name and signature of person dropping off/picking up and date and class child is in.

1240-04-01-.22 Specific Requirements for Child Care Centers

While conducting a QRIS visit, it was observed that a child that was enrolled in the 3-5 year old classroom was in the 2 1/2 - 4 year old classroom. Violation was corrected on site. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Children are to remain in the correct classroom in which they are enrolled.
