During outdoor playtime, a three‑year‑old male student struck a four‑year‑old male student on the head with a brick. — Visit: Technical Assistance,Incident or Injury Investigation — Action taken: The agency is getting fence quotes from several companies and exploring a privacy fence that is 8ft tall in order to prevent items from being thrown over to our playground from the neighbor. Also, they are increasing the playground walk-through to twice a day to ensure that the area is free from debris.

Items labeled "keep out of the reach of children" was out and not lock up — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The director & assistant director immediately removed all items that had"Keep out of reach of children" were removed from classrooms, except for the required spray bottles containing soap/water and bleach water used for cleaning and disinfecting per DHS rules. The Operations Assistant installed cabinet locks in every room; the Director met with each teacher to review proper storage of items listed as "Keep out of reach of children"; and our Assistant Director assisted each class in putting items back in the classrooms behind locks.

The educator was not sitting while the children were eating. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The Director met with Faith and the co-teacher in the assigned room and reviewed mealtime safety procedures. Details were covered, from being seated with the kids to the placement of the teachers while the children are eating. The Assistant Director met with the teaching team and reviewed the importance of staying on the class schedule as posted in the room to avoid mealtime interruptions.
