The incident that occurred on May 7, 2026, was not reported to the parent/guardian no later than the child's release to the parent/guardian, no incident report was done, documentation was not provided to the guardian/parent the same day of the incident, and filed in the child's record, and the child care agency did not notify the Department of the incident the same day of the incident, by emailing or calling the Complaint Hotline or making the submission through the provider portal. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action (s): The agency will begin to complete incident reports and provide them to the parent on the same day that the incident occurs. The agency will also report serious incidents in the provider portal to DHS or DCS. The director will report incidents that occur at the agency when the educators provide the incident report or tell the director on the same day the incident occurs, even if the director has not witnessed it.

The agency will do this by completing an incident report when the incident occurs, notifying the parents about the incident during pick-up, giving the parents the incident report, and placing a copy in the child's file. The agency will also receive additional training through CCR&R on incident reporting.

The director will be aware of any actions taken by any educator that may be considered physically agressive or forceful and report to DHS and the parent on the same day with statements from all parties involved.

The agency failed to report suspected child abuse and neglect to DCS and DHS. — Visit: Complaint Investigation — Action taken: The agency corrected the violation with the following action (s): The agency's director will report any incident of child abuse or neglect to DCS and DHS on the same day of the incident. The director will also carefully consider any action by staff members, whether intentional or accidental, that may jeopardize the health and safety of a child or children.

A substitute educator at the daycare used inappropriate discipline in the classroom while caring for a child. — Visit: Complaint Investigation — Action taken: The agency corrected the violation with the following action (s): The director will take steps to ensure that all educators are familiar with using appropriate discipline in the classroom when caring for children. The director will spend extra time with new employees going over methods of appropriate discipline in the classroom. The director will also observe and visit classrooms to encourage appropriate caregiving and even modeling appropriate interactions and behavior management techniques.

A substitute educator failed to provide daily health and safety to a child in her care in the classroom. — Visit: Complaint Investigation — Action taken: The agency corrected the violation with the following action (s): The agency will make sure that educators provide a safe, healthy, and loving environment for all the children in their care. The director will make frequent visit to classrooms to observe and ensure that the goals and philosophies of the daycare are being met. The director will also make sure that educators provide positive behavior management, responsive and loving interactions and develop behavior strategies when dealing with the children in their care.

The toddler educators were unable to hear, see, or respond to a 2 year old who had been left unattended on the playground. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action (s): The educators were given verbal and written warnings. The director will also monitor and observe the proper use of the name to face roll while eductors are on the playground with the children.

The child was left unattended on the playground without supervision of the educators. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action (s): The educators were given handouts explaining name to face roll call. Also, the director has chosen to monitor the times when classes go out to the playground observing proper use of name to face roll call.

A two year old child was left unattended on the playground reportedly for fifteen (15) minutes. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action (s): The educators received verbal and written warnings. The educators received handouts regarding name to face roll call as well as the director going on the playground with staff to observe proper use of name to face roll call.

The parent was not notified during the release of the child on the day that the incident occurred, nor was an incident report done and placed in the child's file. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action (s): The employee was terminated. The director and staff will complete the training offered by CCR&R. The director will complete the New Director Academy and manage systems and compliance. The staff will also complete training in behavior management.
