LC saw the video and the ratio was 2:7 Children were in the classroom and going outside. There were 2 teachers and name to face was not completed. The teachers went outside and with the kids (6) and 1 child was left in the classroom. The nurse happened to be walking past the classroom and saw him in the room alone. She went in to the class and got him and brought him outside to his class and teachers. -1 teacher was let go this morning due to previous supervision issues, attendance and confidentiality (SK) KH counted the kids but name to face wasn't completed. Name-to-face was not completed before leaving the classroom — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Teachers involved in the violation will complete name-to-face checks using the Procare app before leaving any environment with their students (e.g., classroom, playground) and upon arrival at their new environment. Additionally, they will undergo one-on-one training by an external source on the importance and practice of name-to-face checks. This training will be completed no later than March 14, 2025. All agency employees will complete a two-hour training on active supervision, focusing on key strategies, including name-to-face checks. This training will be completed by March 14, 2025. Additionally, the agency will conduct regular fidelity checks on name-to-face completion in Procare for the next three months.
