A child was left unattended in a classroom. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action(s): Educators immediately informed admin of the serious incident. An incident report was completed for the child and the parents were notified. Admin reported the incident in the Provider Portal, the same day the incident occured. The educators received disciplinary action by the agency stating that the educators cannot be in a classroom alone and must be supervised by other educators.

An internal investigation was completed, and both teachers decided to resign. Moving forward, name-to-faces will be checked by management, and all staff were reminded to focus on their class. Staff were reminded to walk the playground and complete name-to-face to ensure no child is left behind.
