An educator was trying to redirect the child. When the child was redirected, the teacher picked her up by the arm, and the child pulled away and fell to the floor. This incident caused the child to injure her arm. — Visit: Technical Assistance,Incident or Injury Investigation — Action taken: The agency corrected the violation with the following actions: We have moved the educator to an older age group, and she is working under a senior staff member. We have also requested that she undergo further training on how to handle children when they throw fits. The director reviewed the rules with the educator and discussed what to do if she has another child who becomes unruly. The director is reaching out to CCR&R (Child Care Resource & Referral) for further training for all staff members on dealing with unruly children and proper procedures. The director will advise the DHS Childcare Licensing Consultant when CCR&R can accommodate the center with a class.

An educator left three (3) toddlers at the table, unsupervised, eating lunch while she inappropriately disciplined a child for not lying down on his mat. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action(s): The employee was terminated immediately. All staff members were reminded of the importance of proper handling of children and staying seated during meal and snack times.

An educator was observed forcefully placing a child on his/her mat and pushing their head down roughly by the back of their neck. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action(s): The employee was terminated immediately. Management reminded all the staff members of the importance of properly handling children and following the meal supervision plan when children are eating.

The License Consultant verified that all staff members had not completed their required training hours before the licensing year expired. — Visit: Technical Assistance,QRIS Monitoring visit — Action taken: The agency corrected the violation with the following action(s): Management has notified all employees that all required classes must be completed for each DHS calendar year, October 1st through September 30th. We had a staff meeting on November 4th to help reinforce this information.

1240-04-01-.14 Equipment for Children

During the announced visit on 10/21/2024, the Licensing Consultant (LC) observed the pea gravel measurements around the outdoor climbing equipment measured between 0-4 inches in depth. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The Provider have ordered additional pea gravel for the playground. A playground monitor have been instructed to make sure the pea gravel is spreaded each day in the areas needed.

A complaint was made on 9-19-2024 concerning a parent supply list requesting for over-the-counter medication. The medications listed was Tylenol, Motrin and Benadryl. License Consultant (LC) Grady conducted a complaint investigation visit on 9-23-2024 and observed the list posted in the foyer of the child care center with the requested medications observed on the list. — Visit: Technical Assistance,Complaint Investigation — Action taken: The agency corrected the violation with the following action(s): The supply list has been updated by removing the requested medications off the list. Management have communicated with the parents by providing updated documentation concerning administrating medication and emergency situations.
