During lunch, hotdogs were observed to be served, which were cut into round slices, which are potential choking hazards. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Building Surfaces (3280.76)

It was observed that the basement area was accessible to children from the family room child care space and it contained exposed insulation, rust particles on the floor, and an open cat litter box (corrected during inspection). Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. It was observed that the two Pack and Plays were not labeled for the use of specific children (corrected during inspection).
