The emergency plan trainings on file for Staff #2 and Staff #3 were dated 1/22/25 and 2/2/26. These were required to be done annually. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. The health report on file for Child #2, dated 5/29/25, was observed to be missing a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. Requirement: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. Correction: A new health assessment form was provided to parents for childs dr to update. Parent was made aware that a new form was required to remain in care and that the physician must fill out all the required information on the Department's health form, including a statement that the child is able to participate in childcare and appears to be free from contagious or communicable disease. More than 6 months had lapsed since a parent last reviewed, updated, and signed the emergency contact and financial agreement forms. Requirement: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. The emergency contact information on file for Child #5 did not include the parents' work telephone numbers. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Correction: Our Kindertales system has parent work information/ telephone numbers for child #5 listed. Updated paperwork was sent home for families to complete. Inspection: Renewal. Building Surfaces/Clean and good repair (3270.76/3270.102(a))

The Toddler Playground had a shed with a cracked, jagged piece of plywood, which posed a potential scratch hazard. The shed in the Pre-K Playground had cracked siding which also posed a potential scratch hazard. Also, at one corner of the shed, there was an approximately 3.5 inch deep drop off in the grass. This created a potential tripping hazard. Additionally, the Preschool Playground had a Sound Play xylophone with three cracked plastic attachments. In this same playground, there was a cracked clear plastic container and a beach ball with torn fabric. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: All photos of damaged areas were submitted to our government childcare representatives and are now in review to fix. Inspection: Renewal. Locked or inaccessible (3270.66(a))

Imagitarium Beta water conditioner that was labeled, "keep out of reach of children", was observed in a metal cabinet near the sink in the Pre-K2 Room. This item was accessible to older toddler and preschool aged children who were being supervised in that room. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Protective Outlet Covers 5 yrs. or less (3270.65)

Two electrical outlets by the sink in the Pre-K2 Room were observed to be missing protective receptacle covers. These electrical outlets were accessible to older toddler and preschool aged children who were being supervised in that room. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: Missing outlet covers were replaced on 2/19/2026. Staff #2 had not completed mandated reporter training prior to their previous training expiring. This is evidenced by the previously documented mandated reporter trainings on file being dated 8/27/20 and 12/5/25. Staff #4 acknowledged that Staff #2 had routine interaction and direct contact with children during the lapse in trainings. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Fire safety - 1 yr. (3270.31(e)(4)(ii))

The fire safety trainings on file for Staff #2 and Staff #3 were dated 1/9/25 and 2/2/26. These were required to be done annually. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Correction: Staff #2 and staff #3 obtained fire safety training on 2/2/26. No further action is required at this time. Inspection: Renewal. Disposable diapers (3270.135(a)(3))

A soiled disposable diaper was observed in an Older Toddler Room trash can. Staff #3 acknowledged that they disposed of the diaper in the trash can after changing a child. However, the trash can did not have a hands-free lid. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. The health assessment on file for Staff #2, dated 5/4/24, did not include an examination for communicable diseases and the results of that examination. On the health assessment, the physician indicated that they were "not able to assess" for communicable diseases because "vaccine list not provided." Requirement: An adult health assessment must include an examination for communicable diseases and the results of that examination. Correction: This staff member is currently on maternity leave. Upon her return in May, a new health assessment form will be provided and required before returning from leave. Prior to returning to work, this staff will have a completed health assessment that includes all of the required information, such as the results for an examination for communicable diseases. The agreements on file for Child #1 and Child #2 did not specify the date of the child's admission. Requirement: An agreement shall specify the date of the child's admission. Correction: Director checked all files to ensure the date is listed on each agreement on file. Dates were filled in based on admission date for those that were missing. Inspection: Renewal. A cracked orange planter, with sharp edges, was observed on the Preschool Playground. Also, the Infant/Young Toddler Playground had a brown play bridge with multiple cracks. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: Planter was removed from facility playground on 2/24/2025. Brown play bridge was duct taped to cover rough edges on 2/24/2025. Inspection: Renewal. Staff #3 had DHS FBI clearances on file dated 1/21/2020 and 2/7/2025. More than 60 months had lapsed between each DHS FBI clearance. Also, Staff #3 had a Department of Education FBI clearance on file dated 12/30/23, but this clearance is not acceptable for childcare employment. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A staff person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff person #3 had a health assessment on file dated 8/19/24 but it did not include an examination for all communicable diseases and the results of that examination. The staff was only assessed for tuberculosis. Requirement: An adult health assessment must include an examination for communicable diseases and the results of that examination. Correction: Staff person #3 got an updated staff health assessment on October 26, 2024 to show that she does not have a communicable disease. The physician conducted an examination and indicated on the health assessment form that the staff does not have any communicable diseases. Inspection: Unannounced Monitoring.
