On 3/10/25 child #1 slid between the wall and back of the bathroom door and pushed their hand through the door jam and wall resulting in the hand and lower arm becoming stuck. The door was required to be spread apart to free the child. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Door jam pinch guards have been ordered and will be installed on all classroom doors and bathroom doors throughout the center to ensure there are no areas in which the children can access the door jam openings. Durning the unannounced annual renewal inspection on 2/28/25 the inspector observed an Eppi Pen for child #1 and found it to being missing the original container and to have expired March of 2024. Requirement: A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received. Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label. Correction: Immediately following the inspection on 2/28/25, we contacted the parent of the child and explained we needed the correct medication not expired in the original box from the pharmacy or the child could not attend care the following Monday. We received the new medication in the original box which is not expired first thing on Monday morning from the parent. This is now in compliance. Inspection: Renewal. Supervised at all times /Staff assigned to specific children (3270.113(a)/3270.113(a)(1))

During the unannounced visit to the facility on 9/26/24 staff confirmed that on 9/24/24, prior to 9am, two older toddlers left their classroom and the staff who were assigned their supervision. The two children were intercepted by the director in the hallway, just outside the door to the classroom. The children were brought back into the classroom by the director and both staff were asked if they knew the children were missing to which both staff stated they were not aware the children had left the room. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Children must be supervised at all times. Correction: Please be advised that Z & L Holdings Corporation, doing business as Building Blocks Early Learning Academy (hereinafter referred to as Building Blocks ELA), respectfully disagrees with the facts and findings presented in the Inspection Summary dated 9/26/2024-10/1/2024, as well as the alleged violations described as Violation 1 and Violation 2. Additionally, we note that during the initial investigation by the DHS state inspector, it was not communicated that the complaint was verified until days later, by a supervisor who was available only for a virtual meeting and not for an in-person discussion of the findings. On 9/24/2024, a complaint stated that two children were unsupervised at the location. Building Blocks ELA administration promptly conducted its own investigation and found no evidence to support the allegation. Instead, the two children who had exited the classroom were immediately stopped by an administrator stationed outside the classroom door. At all times, the children remained within sight and sound supervision. To comply with the Pennsylvania Department of Human Services, Office of Child Development and Early Learning directive, Building Blocks ELA submits the following Plan of Correction: Effective immediately, all children on the facility premises and on facility excursions will be supervised by a staff person at all times. Staff members will be responsible for the supervision of specific children and will know the names and locations of each child in their assigned group at all times. Staff will be physically present with their assigned children, ensuring constant supervision. Building Blocks ELA's approved practice includes assigning each child to a roll call sheet upon arrival. The child's name is recorded on the roll call sheet, which also lists the teacher(s) at the top. These sheets are monitored throughout the day by a staff member. When a child changes classrooms, their name is crossed off the initial roll call sheet and added to the roll call sheet of the new classroom by center management. Administration monitors supervision throughout the day. Staff are required to identify each child in their group to the administrator conducting oversight. Additionally, staff count children periodically to confirm all children are accounted for. Roll call sheets are available for licensing review upon arrival. Administration also conducts a minimum of three daily classroom visits to verify compliance with child supervision policies and regulations. Building Blocks ELA employs, as a standard practice, two full-time administrators who are always on-site, ensuring that child supervision and operational oversight meet State Licensing regulations at all times.
