The inspector observed incomplete emergency contact information in each childcare space. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: Emergency contact information is housed in each licensed childcare space. The director was unaware that the entire emergency contact form information was required in each childcare space. While at the facility unannounced on 5/7/25 the inspector observed two young toddlers in highchairs without being strapped in as is required. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Disposable diapers (3270.135(a)(3))

While at the facility unannounced on 5/7/25 the inspector observed the toddler room garbage can to be broken rendering it unable to be hands-free. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.
