The inspector observed emergency contact information in each childcare space however the information was missing some of the required information.. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: The emergency contact information in in each childcare space contained the name of the child, DOB, parent's information including addresses and phone numbers, release persons, addresses, phone numbers. The director kept the health insurance information and written consent signed by a parent for emergency medical care in the office, at the facility, in the main child files. Inspection: Renewal. The inspector observed a medication log for child #1 in the preschool room to be missing the child's name and name of the medication the child was prescribed. Requirement: A medication log shall include the name of the medication. A medication log shall include the name of the child receiving the medication. Correction: Once this was pointed out it was immediately corrected. The parent had signed the form and neglected to complete all sections as are required. While at the facility unannounced on 5/28/25 for the renewal the inspector reviewed the staff files and found that staff #1 was hired with a volunteer Pa State Police clearance and volunteer PA Child Abuse clearance. Staff #1also did not submit for the required out of state clearances at the time of hire (see LIS Code Sheet for DOH) as evidenced by the three completed out of state clearances being obtained on 4/15/25, 4/22/25, and 4/16/25. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person # 1 may not work in a childcare position at the facility. If staff are going to be hired under the 45-day provisional hire basis, the following conditions must be met; facility received the results of the applicant's PA Child Abuse History Clearance, NSOR Clearance, applied for and awaiting for all out-of-state clearances (if applicable), a signed disclosure statement, prior to employment and have received the result of the applicant's Pennsylvania State Police OR the FBI finger-print results prior to employment. Proof of submission must be on file for either the FBI or PA State Police Clearance. The employer, administrator, supervisor or other person responsible for employment decisions has no knowledge of information pertaining to the applicant which would disqualify him from employment based on CPSL. Correction: Staff member applied for clearances under "employment" as soon as the clearances were reviewed to be listed as "volunteer" by DHS representative. Both child abuse and criminal clearance were officially submitted on 5/28/25.This employee was placed on a probation period, and they will be able to resume with employment as soon as both clearances are returned with a statement of "No Record".
