    On the day of inspection, toilet bolts were exposed, large shelving was not secure to the wall and the furnace room and supply room were accessible to children. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The toilet bolt was covered, large shelving was removed, and the furnace room and supply room locks were repaired. Inspection: Renewal.     Restraints prohibited (3270.113(e))

    Based on written and video documentation it was determined that staff #1 and #2 restrained a child due to his inappropriate/unsafe acting out behaviors. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. The legal entity will hold a staff meeting to review the citation and program policy in regard to restraining a child. The legal entity will conduct a staff meeting to review the complaint investigation. The legal entity will review the programs policy in regard to acceptable behavior management interventions. Staff #1 and #2 will complete 2 hours of childcare training in regard to behavior management or handling disruptive behaviors. Correction: Staff meetings were held to review the citation, the complaint investigation, the program policy in regard to restraining a child, and the policy in regard to acceptable behavior management interventions. The behavior management plan was modified to include behavior tracking with an emphasis on following the behavior management checklist. In addition to utilizing Rapid Response and IECMH, and positive discipline practices, we have included the use of the Triple P Positive Early Childhood Education program (PECE).
