At that time, plastic bags were observed in a floor level cabinet and in children's cubbies in the first room on the left where they were accessible to the toddlers in care who are still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. A renewal inspection was conducted 4/7/26. At that time, exposed bolts were observed on the fencing at the entrance to the facility. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. A renewal inspection was conducted 4/7/26. At that time, uncovered outlets in a live power strip were observed on the counter in the lunch room where they were accessible to the children. Staff stated that all children with the exception of infants eat in the lunch room. Four uncovered outlets were observed in the room the farthest to the left where they were accessible to the preschool children who receive care in the room. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: Protective covers were immediately placed in all electrical outlets accessible to children under five years of age. Inspection: Renewal. A complaint investigation was conducted on 9/23/25. Through interviews, it was found that Child #4 had been left in a soiled diaper on more than once occasion. The soiled diaper was discovered by the parent when picking up the child to go home for the day. Requirement: A staff person shall check a child's diaper at least every 2 hours and whenever the child indicates discomfort or exhibits behavior that suggests a soiled diaper. A staff person shall change a child's diaper when the diaper is soiled. Correction: Staff was reminded to check diapers every hour for children and change them as needed. A complaint investigation was conducted on 9/23/25. Through interviews, it was found that the following has occurred at the child care center: Staff Person #1 has been heard saying "Eat your food. I'm not having an argument, you can sit in time out all day." Staff Person #1 has kept children in time out for up to 60 minutes for not eating their food and drinking their milk. Staff Person #1 was terminated from employment on 9/18/25. Requirement: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. A child may not be forced to eat food. Correction: Staff Person #1 was immediately terminated due to the violation of policy in employee handbook. Staff will never single out a child for ridicule, threaten harm to a child or a child's family and will never force a child to eat food. Inspection: Complaints- Legal Location. A complaint investigation was conducted on 9/23/25. Through interviews, it was found that the following has occurred at the child care center: Staff Person #1 spanked their own child, Child #2, while saying "I told you I'd hit you if you didn't stop." This occurred at the child care center while the child was in care. Staff Person #1 has yelled aggressively at the children, "Go eat your food" "I'm gonna beat your ass." "I'm not going to deal with your cranky shit today." Staff Person #1 and Staff Person #2 have been seen grabbing children by the biceps, slamming them down into chairs. Staff Person #2 was heard telling a child "I'm going to pull your hair, I'm not playing with you," followed by the child yelling "ow." Staff Person #1 and Staff Person #2 were terminated from employment on 9/18/25. Requirement: A facility person may not use any form of physical punishment, including spanking a child. A facility person may not use harsh, demeaning or abusive language in the presence of children. TIERED LIS 1. This portion of the plan shall have an immediate correction date. 2. The legal entity must create a discipline policy. This policy must specifically include positives strategies for guiding children's behavior. The discipline policy must include what to do if a staff person observes another staff person not following the discipline policy. The legal entity must receive approval of the policy from the Regional Office. Once approved by the Regional Office, the legal entity must review policy with all current staff and have staff sign off that they understand and will follow the discipline policy. All new staff hired must review the discipline policy with the director at their time of hire and must sign off that they understand and will follow the discipline policy. The operator shall provide a correction date for when the discipline policy will be reviewed with all staff. Correction: 1. 9/19/25 Staff will never use any form of physical punishment, including spanking a child. and will never use harsh, demeaning or abusive language in the presence of children. 2. 10/16/25 Policy of discipline of children has already been established and training is done prior to interactions with children. A complaint investigation was conducted on 9/23/25. Through interviews, it was found that children are placed in time out for up to 60 minutes by Staff Person #1. Staff Person #1 was terminated from employment on 9/18/25. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Correction: Staff Person #1 was immediately terminated when director investigated the incident. Staff was reminded of policies. They were trained in and due to violation the employment was terminated. (Employee handbook enclosed.) A renewal inspection was conducted 4/25/25. At that time, the emergency contact form for Child #1 did not include the address of the release person. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Fire drill were conducted on the following dates: 5/1/24, 7/1/24, 8/1/24, 8/28/24, 10/23/24, 12/17/24, 2/27/25, 4/2/25. Fire drills had not been conducted within 60 days on two occasions. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. A renewal inspection was conducted 4/25/25. At that time, the Harmony booster seats in the facility vans stated that use should be discontinued after 2022. The instruction manuals for the booster seats was not found in either van. Requirement: A child 7 years of age or younger shall be transported in accordance with the requirements for parents and guardians as stated in 75 Pa.C.S. §4581 (relating to restraint systems). Manufacturers' instructions for use of safety restraints shall be kept in the vehicle at all times. Correction: New booster seats will be placed in each van and the corresponding instruction manuals will be kept in the vans. Inspection: Renewal. A renewal inspection was conducted 4/25/25. At that time, the facility was using Learning Beyond Paper to complete assessments on the children. Learning Beyond Paper is not an approved assessment through OCDEL. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). At that time, exposed bolts were observed on the fencing of the school age playground, the young school age playground, the preschool playground, and the toddler playground. A basketball hoop was observed on the young school age playground. The weighted portion of the basketball hoop contained green, standing water. The spaceship climber on the toddler playground was observed to be cracked on the slide posing a pinch hazard. Small debris was trapped underneath the mats on the toddler playground where the play equipment was located. The debris caused the mats to be uneven, posing a tripping hazard to the toddlers. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The bolts will be cut or covered. The slide will be repaired. The debris will be swept out from under the mats. Inspection: Renewal. A renewal inspection was conducted 4/25/25. At that time, a hole was observed in the wall under the chalkboard upstairs. The molding under the door of the middle school age room was cracked with a nail sticking out. A broken board with sharp screws was observed on the school age playground. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The hold was plastered and repaired during the inspection. The molding was replaced during the inspection. The board will be repaired. Inspection: Renewal. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Staff person #1 must complete PA Required Health and Safety for Centers-Group Child Care Parts 1 and 2.
