The immunization records on file for child #1 were not in accordance with the schedule recommended by the ACIP. The record did not include documentation of the 2nd age appropriate Hepatitis A dose. The child was in care at the time of inspection. Requirement: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings). Correction: We told the parent what the child #1 was missing and the child was not allowed at the daycare until he got the vaccine needed and brought the updated health report with the missing Hep A dose. Information about the following topics is not being given to parents in writing at the time of enrollment: general daily schedule, meals, clothing, supervision policies, dismissal policies, and pick up arrangements Requirement: At the time of enrollment, a parent shall receive in writing the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements. Correction: We include the topics mentioned above in the contract we give the parents at the moment of enrollment. Heat Source (3280.71)

The baseboard heater in the child care space measure up to 127° F. The heater in the children's bathroom measured up to 130° F. Both of the heaters were accessible to children. Requirement: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact. Correction: We lowered the temperature at the moment it was measured. We have covers for the baseboard heater if its necessary to rise the temperature of the heaters. Inspection: Renewal. Locked or inaccessible - in original container (3280.66(a))

Toxic materials were observed underneath the sink in an unlocked cabinet in the children's bathroom. The cabinet included hand sanitizer, hydrogen peroxide, bug spray, and pet shampoo (labeled to keep out of the reach of children). Hand sanitizer was observed on a shelf in an area accessible to the children. (CORRECTED ON SITE) Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Correction: We installed locks to the doors in the bathroom cabinet, avoiding the children to have access to the products inside it. Hand sanitizer was removed from the reach of children. Uncovered electrical outlets were observed in the children's bathroom and in the hallway outside of the children's bathroom. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: Protective electrical covers were placed in the children's bathroom and in the hallway outside the bathroom. Inspection: Renewal. Upon inspection of children's files, it was found that for Child #8, the agreement form did not include a fee amount for services. Requirement: An agreement shall specify the amount of the fee to be charged per day or per week. It was observed that the facility tested smoke detectors on 11/01/24 and then again on 12/02/24, (31 days lapsed) and 12/02/24 and 1/03/2025 (32days Lapsed) which is greater than the allowable 30 days between testing dates. Requirement: The primary staff person or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. The primary staff person or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Correction: we will plan to test the smoke detector every 30 days, we didn't knew that we could do it on weekends so we will make sure to test the detectors every 30 days or if it is necessary less but no more than 30 days Inspection: Renewal. Upon inspection, it was observed that staff person #2 did not wash the hands of an infant child after diapering. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Correction: we install a sink next to the changing table so we would de able to make it easier to wash hands of staff and children after diapering, toileting and before meals and snacks Inspection: Renewal. Upon inspection of children's files, it was found that for Child #5, the latest health report was dated 7.16.24 did not include an update. ( within 6 months). Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. Upon inspection of children's files, it was found that for Child #1 and Child #2, the emergency contact form did not include the release person's complete address. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Correction: we got in contact with parents of child#1 and child #2 to get the information that was missing the same day of inspection and complete the file Inspection: Renewal. Upon inspection of children's files, it was found that for Child #6, the emergency contact form did not include Medical Insurance policy number. Requirement: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. Correction: I text mom of child #6 at the moment of the inspection to get that information and she did give it to me so we completed the file the same day of inspection Inspection: Renewal. Upon inspection of children's files, it was found that for Child #1 and Child #4, the emergency contact form did not include the enrolling parent's home address. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Correction: it did have the parents home address, it was just missing town and state, it was incomplete. we ask for the information and completed the parents address on file Inspection: Renewal. Upon inspection of children's files, it was found that for Child#7, the agreement form was an original rather than a copy.. Requirement: A parent shall receive the original agreement. The facility shall retain a copy of the agreement. A child's record must contain a copy of the initial agreement and subsequent written agreements between the parent and the operator. The parent receives the original agreement. Upon inspection of children's files, it was found that for Child # 6, the agreement form did not include a date of admission.. Requirement: An agreement shall specify the date of the child's admission. A child's record must contain the dates of application, admission and withdrawal of the child. Upon inspection, it was observed that rest equipment used in the facility had less than 2 feet on 3 sides while in use during nap time. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: we remove equipment that were closer than 2 feet apart so there will be enough space between them during nap time Inspection: Renewal. Upon inspection of the facility, it was observed that 1 infant child and 3 younger toddlers were in swings and not using proper rest equipment. The rest equipment used in the facility were not labeled for the use of specific children in care. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: we remove the children from the swings at the moment of the inspection and will maintain it that way during their nap time, they do have their rest equipment identified with their names or numbers for each one and they are all individual, age appropriate and clean for each child Inspection: Renewal. Upon inspection, it was observed that protective electrical covers were not placed in the by one electrical socket. (CORRECTED ON SITE). Correction: It was corrected on site, we proceed to cover the electrical outlets with the electrical covers to all the outlets that were accessible to children 5 years of age or younger. Primary staff person present/Mixed age levels (3280.52(a)(1)/3280.52(c))

Upon inspection of the facility, it was observed that there were 9 children napping with one staff person present. There were 2 infants, 4 young toddlers, 1 older toddler, and 2 preschool children with 1 staff person. The operator was not present in the facility at the initial time of the renewal inspection. Requirement: A primary staff person shall be present in a facility when six or fewer children are in care, unless the staff-child ratio specified in §3280.52(b) or (c) requires a second or third staff person. When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements at §3280.52(b). The file for household person #3 contained a NSOR verification that was dated 3.3.20 [expired on 3.3.2025]. The facility had no record of an application for the updated clearance on file. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Household member #3 must be removed from the facility by close of business during operation hours when children are in care [effective 3.25.25]. All required clearances must be obtained before household member #3 may return to the facility during normal childcare hours of operation.
