The EPI Pen and Albuterol inhaler was observed without manufacture's or health professional's instructions on the original label. Requirement: A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received. Staff #1 was assigned to classroom #104 where 11 children (ages 1--2-year-olds) had 2 staff members exceeding the ratio of 1:5. Staff # 2 and 3 were assigned to classroom #104 where 6 infants were alone to 1 staff member exceeding the ratio of 1:4. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff: child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Children must be supervised at all times. Each first aid kit taken on an excursion must contain a bottle of water in addition to the items specified at §3270.75(c). When children leave the facility on walking and riding excursions, emergency contact information specific to each child on the excursion shall accompany a staff person on the excursion. Observed 7/28/2025 in classroom 9/11 the wall in the back in the classroom space where the electrical outlets were there was wires exposed where children hands could easily touch and make it not free from visible hazard or good repair. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
