Observed 11/19/2025 in Infant A classroom there were Clorox wipes located in the childcare space not locked and inaccessible to children. Classroom 114 had Wet One wipes located in the childcare space not locked and made inaccessible to children. Classroom 212 there was Febreze spray located in the childcare space not locked and made inaccessible to children. In the girl's bathroom between classroom 201 and 204 bathroom there were baby wipes labeled "Keep out of Reach of Children" located in the toilet stalls on the toilet not locked and made inaccessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Observed 11/19/2025 in classroom 215 under the sink there was evidence of infestation of dead insects where children complete handwashing where it can be accessible of children placing their hands under the sink to touch. Requirement: Evidence of infestation of insects or rodents in the facility is not permitted. Correction: Glue trap removed from under the sink and placed out of reach/sight. Inspection: Renewal. Observed 11/19/2025 classroom 215 were missing items from their first aid kit. Classroom 215 was missing tweezers and soap from their first aid kit. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Observed 11/19/2025 staff #1 health assessment did not contain the physician's address of where their health assessment was conducted at. Staff #1 TB test did not include the date the test was read. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. Observed 11/19/2025 staff # 3 was observed leaving children unsupervised in the bathroom alone in the hallway not able to hear, see or access the children's whereabouts. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Observed 11/19/2025 in classroom 204 the refrigerator where children lunches and snacks are stored the door handle had sharp edges and ridges that was not in good repair and free from visible hazards. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Our building carpenter repaired the sharp edges and ridges on refrigerator and filed down sharp edges and placed an adhesive felt pad to protect the surface as a temporary measure. Observed 3/6/2025 in classroom 201 and 215 the walls boarders in the classrooms were dislodged from the wall and classroom 201 had peeling paint crumbling making it free from visible hazards., good repair and accessible to children Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Wall boarder in room 215 was fixed to the wall. Observed 3/6/2025 Child # 1 Inhaler in 215 were not in its original container and was placed in a zipped locked bag without manufacture's or health professional's instructions on the original label. Requirement: A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
