It was observed that an unlocked cabinet in the pre-k room contained mod podge, shaving cream, and rubbing alcohol. In the outdoor play area, which was not in use, a container of weed killer was observed in one of the swings and a bottle that said all-purpose clearer on it was in reach of the children. Along the walkway to the playground was an aerosol can and cigarette butts. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Correction: A Lock was immediately installed on the cabinet in the Pre-k Classroom to ensure all cleaning supplies and potentially hazardous materials are inaccessible to children. The classroom and outdoor play were thoroughly inspected, Cleaned, and all prohibited items, including the weed Killer Container, aerosol can, Cigarette Butts, and unlabeled bottle, Were Remove. It was observed that there was trash in an unlined trash can in the pre-k room. (Corrected on site.) Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Correction: The Trash can in pre-k classroom was immediately lined with a plastic trash bag, and all waste was properly disposed of in accordance with licensing Regulations. It was observed that the gate leading to the kitchen had a broken latch, which made the stove accessible to children. Requirement: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact. Building Surfaces (3280.76)

In the pre-K classroom, which was not in use at the time, it was observed that the children's couch was ripped. The classroom shelves were excessively dirty; a broken table and a broken gate were in the care space. In the outdoor play space, there was a piece of broken fence was rusty nails sticking out of it, trash on the edges of the playground, moldy burlap sacks, excessive dirt build-up on the Little Tykes car, and there was broken glass along the outside walkway to the playground. There were an excessive number of weeds on the perimeter of the playground. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The identified hazard was immediately removed/repaired, and the affected area was cleaned to ensure the environment was safe for children. Stairs, exits, etc. unobstructed (3280.91(a))

It was observed that the door in the storage room behind the kitchen that leads to the playground was blocked by furniture and boxes. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: All Furniture boxes, and other items blocking the exit door in the storage room were immediately removed. The exits pathway was cleared to ensure the door is fully accessible and unobstructed for emergency egress. Inspection: Allocated Unannounced Monitoring. Clean and good repair (3280.102(a))

It was observed that the tic-tac-toe game on the climber had multiple broken pieces causing rough edges that could result in a pinch point. There was a plastic container that was cracked and could result in a pinch point. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The Damaged tic-tac -toe game on the climber and the crack plastic container were immediately removed from use to eliminate pinch-point hazards. The playground and equipment were inspected, and the any damaged items were repaired or remove to ensure the safety of children. Inspection: Allocated Unannounced Monitoring. It was observed that the fire drill log did not state that fire detection system was sounded for the fire drills and there was not a separate log for testing. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The primary staff person or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met Correction: The fire drill log was updated to documents that the fire detection system was sounded during fire drill.
