Building Surfaces (3270.76)

It was observed that the ground covering under the mulch on the preschool playground was sticking up through the mulch in various causing a tripping hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Documentation showed the staff person #3 had a 21-day lapse in their mandated reporter training. 5/22/20- 6/12/25 Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Fire safety - 1 yr. (3270.31(e)(4)(ii))

Documentation showed that staff persons #1, #2, #3 and #4 did not have fire safety training annually. They had it on 1/27/25 and 2/10/26. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Training regarding plan (3270.27(c))

Documentation showed that staff persons #1, #2, #3 and #4 did not have emergency plan training annually. They had it on 1/27/25 and 2/10/26. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. 1.) During the inspection, ceiling tiles in the facility kitchen above the sink area were observed to be missing or in disrepair. 2.) The edge of the surfacing of the outdoor toddler play space was observed to be at a height that constituted a tripping hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The ceiling tiles in the kitchen were replaced. There are no signs of leaking above the tiles. The fence in the toddler play area has been moved so that there is no longer a gap between the edge of the surfacing and the fence. Inspection: Renewal. At the time of the inspection, documentation showing that the emergency plan had been sent to the local municipality was not available. Requirement: The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency. Correction: An updated emergency plan was emailed to the Lancaster County EMA and West Hempfield Township EMA through the email addresses provided by the Lancaster County EMA. Inspection: Renewal. The custom form did not allow space for parents to indicate their written consent for these items. At the time of inspection, documentation of a release person's phone number and address was not available for child #6. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Correction: Director, Samantha Donley, spoke with Child #6's mom and emailed for an updated contact information.
