It was observed that the ground covering under the mulch on the preschool playground was sticking up through the mulch in various causing a tripping hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
