Per video footage review, Staff person #1 remained sitting in a chair while in the playroom with 3 OT children and was not physically present with Child #1 and #2 and therefore unable to intervene when Child #1 began to bite Child #2. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: After viewing the video footage, staff person #1 was suspended from working with the children. All staff were reminded of the importance of supervising properly. Per video footage, Staff person #1 was observed viewing their cellphone while in the playroom with 3 OT children, and was not adequately supervising the children, leading to Child #1 biting Child #2 in the back several times. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: After viewing the video footage, staff person #1 was suspended from working with the children. All staff were reminded of the cell phone policy currently in place and that supervision of the children involves being able to see, hear, direct, and assess. Inspection: Complaints- Legal Location. Health and Safety Training - Pediatric First Aid and CPR (3270.31(f)(10))

Staff person #1 has pediatric first aid and CPR training that is dated 8/23/25, which is more than 90 days after their date of hire (see LIS code sheet). Requirement: Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.
