The facility did not have a written policy to address the following: * Recognition of potential signs/symptoms of shaken baby syndrome and abusive head trauma * Strategies for coping with a crying, fussing or distraught child * The prevention and identification of child maltreatment Staff Person #8 was reportedly caring for children unsupervised and had not completed the following pre-service training prior to caring for children unsupervised: * Pediatric First Aid/CPR * Health and Safety topics * Mandated Reporter training Staff Person #7 has not completed the following required pre-service training within 90 days of their date of hire: * Health and Safety topics (completed only the 1-hr 2022 update with no record of the original 10-hr "Get Started with Center-Based Care: Building Blocks for Quality) Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre-service trainings are completed, Staff Person #8 must be supervised, when interacting with children at a minimum by an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff persons available to supervise Staff Person #8, Staff Person #8 may not work in a childcare position at the facility. Staff Person #7 must complete PA Required Health and Safety training for Centers-Group Care Parts 1 and 2, unless documentation of the original, 10-hr training is received and on file. Correction: A policy regarding Shaken Baby/Child Maltreatment was created and added to the Parent Handbook. All Staff signed off on an Acknowledgement form to indicate that they are aware of the policy. Staff Person #8 was removed from a childcare position until all trainings and complete staff file items were completed and documented in the staff file. Training regarding plan (3270.27(c))

There was no documentation of the annual Emergency Plan Training having been done for Staff Persons #1, 2, 3, 5 or 6. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. Correction: Staff Person #6 is no longer employed at the facility. The Director trained Staff Persons #1, 2, 3 and 5 on the Emergency Plan. Sign-off sheets of the training are in the staff files. Fire safety - 1 yr. (3270.31(e)(4)(ii))

There was no documentation of annual fire safety training completed for Staff Person #3. The last documented training was on 10/14/24. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Facility Person #8 began working with children unsupervised, before the provisional hire clearances and Disclosure Statement were received and on file as follows: * Child Abuse Clearance was not completed and dated until 1/5/26, after the start date (see CODE LIS sheet). * The DHS FBI Clearance was not on file (nor was proof that the fingerprint clearance had been submitted), until 12/17/26, after the start date. * There was no dated, signed and witnessed Disclosure Statement on file. Staff Person #7 had clearances with criminal charges, that had not been submitted to the certification department for review and approval. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 8 may not work in a childcare position at the facility until the Disclosure Statement (and complete staff file items) are on file and reviewed by cert rep. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #8 did not have a staff file that included verification of childcare experience, education and training prior to driving children to/from school. Staff Person #8 was not supervised by another staff person, therefore was working as an Assistant Group Supervisor without the required documentation for that position. Staff Person #4 did not have documentation of Education to work as an Assistant Group Supervisor on file. Requirement: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing. A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Correction: Staff Persons #8 and #4 will not work unsupervised with children until proof of education and childcare hours of experience have been obtained and documented in the staff file, that qualify Staff Persons #8 and #4 as an Assistant Group Supervisor. Diaper creams and hand sanitizer were observed to be accessible to children, on a low shelf by the sink in the Bees Classroom (corrected on-site). Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Diaper creams and hand sanitizers were immediately removed and placed in a locked cabinet, out of reach of children. Inspection: Renewal. Emergency telephone numbers were not posted by the phone in the Caterpillars Room. Requirement: The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility. No peeling paint or plaster (3270.77(a))

Peeling paint was observed on the wall under the bulletin board and radiator in the Prekdg Classroom. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Children were removed from the space immediately and transferred upstairs to a new classroom space to occupy until the work was completed. This scheduled project to fix the peeling paint in the classroom, will be completed by maintenance and owners' involvement to correct citations. On 3/19/26, the areas of peeled paint were covered with butcher block paper as a temporary correction, until the room has been repainted by maintenance. Inspection: Renewal. Nap mats were not labeled for the use of specific children and used only by the specified child in the Caterpillars Room. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: Nap mats were labeled immediately by using a nap chart in the classroom with each child's name and number assigned. Inspection: Renewal. Staff Person #8 has been driving children to/from school without a copy of their valid driver's license. Requirement: The operator of the vehicle shall be 18 years of age or older and shall have a valid operator's license. There was no staff file for Staff Person #8 that included an initial health assessment and TB test results conducted within 12 months prior to initial service in a childcare setting. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Refrigerator (3270.107)

The thermometer in the Caterpillar Room's refrigerator was showing a temperature of 50 degrees F. The door had been left ajar. The refrigerator was re-checked an hour later and the temperature on the thermometer was still showing a temperature of 50 degrees F. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: The thermometer was replaced immediately, and the door was secured with a lock to ensure the proper temperature was maintained throughout the day. Inspection: Renewal. The files for Staff Persons #1 and #3 did not include documentation of the required childcare experience for the position they were working in the capacity of. There was no written consent from the parent of Child #1, to administer medicine at the facility. There was no Medication log for Child #1's medication at the facility. The medication log must include all of the information outlined in 3270.133(7) (i through viii). Requirement: A parent shall provide written consent for administration of medication or a special diet. An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered. Correction: A new med log was provided to staff to have parents complete and sign to keep with the medication and a complete medication log is at the facility with the medication.
