Requirement: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Correction: Mid Year agreements and updates were sent to parents on the day of inspection. Correction: The family was due on December, and did not get an appointment until January due to holidays. The family has since updated their Health form in January. Staff person #1 has been working in classrooms without the required out-of-state clearances on file. Staff person #2's current Child Abuse Clearance results were not on file during the inspection. Small Toys and Objects (3270.103)

It was observed, in the Toddler room, that small pom-poms and plastic hearts (less than 1 inch diameter) were accessible to children in a drawer to the left of the sink (corrected during inspection). Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Inspector had to remove dust pan/broom and chair to open the drawer herself. The drawer was not accessible to a Toddler. It was observed that the outdoor play area is located right by the facility parking lot. Toddlers could easily run into the parking lot. Requirement: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions. Child #1's Child Service Report (providing information to the family about the child's growth and development) was not completed on our department's form or a form approved by our department. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). Correction: NSM will use an approved Service Report for children's growth and development. The AMS child service report is much more detailed and provides the exact referenced materials as the service report the State department has provided. Inspection: Renewal. The time between staff person #3's health assessments exceeded the permitted 24 months' timeframe (5/19/22, 8/8/24). Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Correction: Provider disagrees with this violation--as staff person was on maternity leave during the required time for her to get an updated health form. Staff person did get health form completed prior to returning to work. NSM will collect a health assessment every 24 months with a date and signature from staff. Inspection: Renewal. Staff person #2's file did not contain proof of obtaining 12 annual clock hours of child care training. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. Correction: Staff person is completing Montessori Training credits and was not able to give explanation of courses in the amount of time requested. We will make sure this is in her file. It is unknown if child #1's initial health report and immunizations were obtained by the facility within the child's first 60 days of attendance due to the health report not being dated. See Code Sheet Requirement: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. A health report shall include a review of the child's immunized status according to recommendations of the ACIP. Correction: NSM will request updated health form to prove the health forms (child #1) were collected in a timely manner prior to the child starting.
