As reported by the facility, on 1/8/2026, child #1 was given a food that was not permitted as part of the special diet prescribed for child #1. Staff #1 provided this food accidently. Requirement: If a special diet is prescribed for a child and if the diet is administered to the child, written instructions and the parent's written consent shall be retained in the child's file. Correction: We removed the teacher from the room immediately and permanently. We moved the [teacher]to a different room with a different classroom. The teacher was given additional training on food allergies. We had a staff meeting and reviewed all allergies that are in our school. We met with the child's parents and they provided us with instructions on administering special diet restrictions.
