On 2/17/26, at time of inspection, a door leading directly to a stairwell was observed open and accessible to children, creating a potential fall hazard. Requirement: If a door or doorway opens or exits directly into a stairwell and if there is no landing beyond the door or doorway, the door shall be restricted from opening or shall be removed and a secure barrier to prevent access to the stairwell shall be erected. Correction: The door was closed and secured immediately upon discovery. Staff were reminded immediately of the importance of keeping the door closed. Inspection: Renewal. On 2/17/26, at time of inspection, children in the preschool and older toddler rooms were observed utilizing rest equipment labeled for other children, failing to ensure use of individually assigned rest equipment. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: Staff were reminded of state regulation regarding rest equipment assignment.
