Continued noncompliance from inspection conducted on 2/3/26: On 2/20/26, at time of inspection, Child #1 was observed unsupervised asleep on a rest mat on the floor in the back half of the infant room, positioned in the corner directly behind a half wall. Child #2 was also observed unsupervised asleep in a crib behind a half wall. Staff #1 and Staff #2 were located in the front half of the room, preventing proper supervision of Child #1 and Child #2. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: 1. 2/20/26 Children will be supervised at all times. 2. 4/3/26 All staff working in the infant room for any duration of time will receive a minimum of two hours of training in supervision. On 2/3/26, at time of inspection, medication (inhaler) was observed accessible to children in an open backpack located in the older toddler room. Requirement: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children. The staff person shall know the names and whereabouts of the children in his assigned group. On 2/3/26, at time of inspection, the exit door located in the back of the facility was obstructed by snow and not able to be opened in case of emergency. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. On 2/3/26, at time of inspection, lidded waste receptacles were not observed by the toilet/sink area located in the K room and PS rooms. Requirement: A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle. On 2/3/26, at time of inspection, Staff #1 file contained a PA Criminal clearance and NSOR clearance dated more than 60 months. The PA Criminal clearance was dated 10/10/19 and the NSOR clearance was dated 9/1/20. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1- may not work in a child care position at the facility. Correction: Staff #1 PA Criminal Clearance was processed immediately on 2/3/26. Staff #1 submitted an NSOR clearance and is awaiting results. In the meantime, Staff #1 will not work in a childcare position at the facility until results of NSOR are obtained. On 6/26/25, at time of inspection, the EpiPen belonging to Child #1 was expired. On 6/26/25, at time of inspection, the EpiPens belonging to Child #1 and Child #2 were not stored in the original box. A prescription was not on file for Child #1 EpiPen. Additionally, medical action plans pertinent to diagnosis and treatment in case of emergency were not on file for Child #1, Child #2 and Child #3. Requirement: A health report shall include a statement of the child's medical information pertinent to diagnosis and treatment in case of emergency. A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received.
