At the time of reviewing the emergency plan, there was not documentation the plan was sent to the local municipality and county EMA annually. Requirement: The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency. Correction: My emergency plan has been sent via email to my local municipality (Ridgway Township) and Elk County Control. The file of staff #1 had a late health assessment due to the fact they were dated: 9-15-25 and 8-1-23. Requirement: A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Written emergency plan posted (3290.124(d))

At the time of the inspection, a written plan which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency was not posted in a conspicuously location. Requirement: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children. Correction: I will write a plan that covers who will accompany a child in the case of an emergency by ambulance and who will stay back with the children. Clean, age appropriate (3290.105(a))

At the time of the inspection, the rest equipment was not labeled for each child. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. The rest equipment must be labeled with the child's name and used only by the named child while enrolled in the program. Correction: My plan of correction will be to post a diagram that identifies the hung nap mats and give a short description of the nap mat along with the child's name. Inspection: Renewal.
