The file of staff #1 had late NSOR clearances due to the fact they were dated 9-15-25 and 7-7-20. The file of staff #2, had late clearances due to the fact they were dated 11-15-21 to 5-31-16 PSP, 11-16-21 to 5031016 Child Abuse, 11-16-21 to 6-16-20 FBI; and 9-8-25 to 7-7-20 NSOR. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. Observed a wooden letter block with splintering wood. The files of children #2, #3, #4, and #5 lacked child service reports completed every 6 months. Requirement: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e). Based on the smoke detection testing log, there was insufficient documentation to show the smoke detection system was tested more than 30 days apart because an invalid date was documented: 6-31-24. The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in § 3290.95(a) are met. Upon arrival, observed a lack of usage of an electronic monitor, camera, mirror, or other device or method so the operator can see, hear, direct and assess the children in real time at all times. Requirement: When there is only one operator present in the family child care home and the operator is unable to be physically present with the children because the operator is preparing snacks or meals for children in care or the operator is using the restroom, the operator may comply with the supervision requirement in subsection (a) through the use of an electronic monitor, camera, mirror, or other device or method so the operator can see, hear, direct and assess the children in real time at all times. Correction: The monitor used for safety of kids while unable to be physically present when answering door, or bathroom was placed on counter along with handheld device immediately. Emergency plan/Training regarding plan (3290.24(b)/3290.24(c))

Based on documentation, the emergency plan was reviewed more than 12 months apart, 8/2023 and 9/2024. Requirement: The operator shall review the emergency plan at least annually and update the plan as needed. Each review and update of the emergency plan shall be documented in writing and kept on file at the facility. The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training.
