In Room 11, an albuterol inhaler for child #1 was not in the original box and did not have written instructions for administration from the individual who prescribed the medication. Instructions for administration contained on a prescription label are acceptable. Correction: The child's parent sent a picture of the medication box that contains the prescription and directions for administration. The picture was taped to the medication. Inspection: Renewal. The health report of child #1 did not include a statement that the child received all age-appropriate screenings recommended by the American Academy of Pediatrics. Requirement: A health report shall include a statement that age-appropriate screenings recommended by the American Academy of Pediatrics were conducted since the time of the previous health report required by this section. Correction: The health report was updated by the child's pediatrician to include the box that they received all age-appropriate screenings. Inspection: Renewal. Clean and good repair (3270.102(a))

The back corner of a plastic cart, which was attached to a play tractor in Room 11, was broken presenting a hazard to the children due to exposed rough, and sharp edges. In Room 13, a red metal mesh basket was found with fraying along the top rim causing a sharp piece of metal to protrude upward and created a potential puncture hazard to the children. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Building Surfaces (3270.76)

In Room 109, a locker was in disrepair and a potential hazard. The locker's lift handle was missing, leaving the metal attachment point protruding approximately .75 inches from the locker door, creating a risk of injury to children. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Staff member immediately covered the metal piece. No children were in danger as the room was closed and not being used. Handle will be replaced Monday. Inspection: Renewal. Emergency Numbers Posted (3270.74)

The telephone numbers of the nearest hospital, police and fire departments, ambulance and poison control center was not posted at the telephone in Room 9. Current mandated reporter training for staff #2 was dated 11/21/2025, which was more than 60 months from the prior training dated 8/22/2020. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: Staff updated their mandated reporter as soon as they realized the one on file was expired. It was renewed before inspection. Inspection: Renewal. The emergency contact information of child #1 did not include the home telephone number of the enrolling parent. In Room 104, the first aid kit contained 2 single use packets if Neosporin which were not labeled with the name of the individual for whom it was intended. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: Room 104 was a CLOSED room; no children were at risk at any time. In addition, the first aid kits are always locked up. Ms. Taylor disposed of the Neosporin immediately during the inspection. The immunization records of child #3 and #4 are not in accordance with the recommendations of the ACIP and an acceptable exemption is not on file. Child #3 is lacking documentation of 1 of 4 pneumococcal and child #4 is lacking 1 of 2 Hep A vaccinations. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings). Correction: Child #3 immediately spoke to the pediatrician who stated the child is on a medical plan; she was ill when she would have received the missed dose. The pediatrician sent a medical plan. The pediatrician scheduled an appointment; the child received the missed dose on 12/26/24. Child #4's parent provided an updated immunization record that noted both Hep A vaccinations. In Room 11, where infants and toddlers were receiving care, foam pool noodles approximately a foot long each were stored in a bin on a low shelf accessible to children still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: The pool noodles were used for supervised activities. Ms. Taylor immediately threw away the pool noodles. Inspection: Renewal. Running water - diapering (3270.82(j))

The diapering area in Room 12 did not have a source of running water for handwashing. Requirement: A source of running water for handwashing shall be present in infant and toddler diapering areas. If the running water does not flow directly into a drain that is connected to a sewage system, a receptacle shall be provided to contain the water used for washing. The receptacle shall be emptied into an approved sewage system at least once a day. Correction: During inspection, the portable sink in the Nemo room became clogged with formula. Ms. Taylor grabbed the sink in the next room (this room is CLOSED and has no children). The maintenance man cleaned out the sink immediately and all sinks are working. Inspection: Renewal. Heat Source (3270.71)

When measured, the radiators in the following areas had a temperature of at least 130 degrees Fahrenheit and were accessible to children: Room 11, Room 12, Room 13, Room 103, Room 104. Room 105, Room 106, Room 107, Room 108, Room 109, the 2 restrooms by Room 107, and in the hallway of Rooms 11, 12 and 13. In the Multipurpose Room, the hot water pipe which runs along the bottom of the entire wall and part of another, when measured, had a temperature of more than 140 degrees Fahrenheit. The hot water pipe in the corner of the girl's restroom downstairs, was not covered in an area that was accessible to the children and when measured, had a temperature of approximately 161 degrees Fahrenheit. Requirement: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact. Correction: A heating engineer was immediately called in, and his infrared thermometer did not read over 110 degrees on the surfaces the children have access to. Inside the vents (which are too small for little fingers) read above 110 degrees. In Rooms 11, 12, and 13, a barricade was immediately erected so children cannot get close. All other rooms were locked since they are CLOSED and do not have children. The hallway radiators were turned down. Additional protectors were ordered and will be installed on January 6, 2025. The hot water pipes (along the floor) in the multipurpose room were immediately wrapped 12/17/2024. The hot water pipe in the corner of the girl's restroom downstairs was immediately wrapped when the inspector was there. 12/16/2024. Inspection: Renewal. Locked or inaccessible (3270.66(a))

Room 104 contained 3 12-ounce bottles of "Hand 2 Mind Iron Fillings." In the back of the of the room on a desk of materials were 2 bottles and in the front of the room was 1 bottle of the iron fillings in a box on the floor. Each of the bottles, which were accessible to children, had a safety data sheet attached with a rubber band which included to keep out of the reach of children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Room 104 was CLOSED and not in use; NO CHILDREN WERE AT RISK. We had a science lab donation; we did not have a chance to go through the boxes of donations.
