A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Clean and good repair (3270.102(a))

In the outdoor play space, the adhesive repair to the platform of a toddler slide was pulling off exposing a cracked area approximately 2 inches in length. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Handwashing signs (3270.82(h))

The training chair in the restroom did not have a posted handwashing sign. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: The training chair was removed as it was not used. The drawstring bags were hanging on the door handles in each of the outdoor play spaces at a height which made the first aid kits stored inside the bags accessible to children. Requirement: A first-aid kit must be inaccessible to children. Correction: Hooks were placed outside so that the first-aid kits can be hung on them when outside. The file of staff #2 contained documentation of 9 training hours in their annual training year of December 2022 to December 2023 lacking 3 hours to meet the annual 12-hour requirement. Staff #2 had taken 2 hours of training on 2/19/2024 and 1 hour on 6/5/2024 and is now in compliance. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility. Correction: Staff #2 had additional training hours for the year 2024 that were used to meet the additional 3 hours needed for 2023. Staff #1 is now in compliance. Inspection: Renewal. Staff assigned to specific children (3270.113(a)(1))

Durning rest time, staff #1 was asked to identify the children in the primary supervisory group of the 7 children in the infant classroom. Staff #1 identified 3 children (2 infants and 1 young toddler). Staff #2 was then asked, and they identified 4 children (all under the age of one year) in their supervisory group. One of the children identified in the supervisory group of staff #1 was also identified by staff #2 leaving one child, who was asleep in a crib, not identified by staff #1 or staff #2 Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff #2 was told she was responsible for the remaining infants that the other staff in the room did not identify, this included the infant sleeping in the crib. Inspection: Renewal.
