Maintenance worker was observed to be utilizing the children's restrooms while children and staff were present at the center instead of utilizing the adult/staff restroom. During the investigation, it was verified through staff interviews that Facility Person #1 glued down a piece of bumper with liquid nails adhesive in the Chipmunks Room while children were in care. During lunchtime clean-up, Child #1 pulled the bumper off, put their fingertip into the substance and licked it. Requirement: Cleaning materials and other toxic materials shall be used in a way that does not contaminate play surfaces, food, food preparation areas and does not constitute a hazard to the children. Correction: Facility person #1 was instructed he is not to use any material listed as, "do not use around children, not safe for children or keep out of reach of children" or any like description in a classroom during classroom hours where children are present or may be present during the timeframe that the product is deemed unsafe.
