During the renewal inspection, the health assessment for Staff #1 was dated 2/10/25 which was more than the required 24 months from the previous health assessment dated 9/14/22. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Correction: It was determined this staff person did have a physical 12/2023 but did not have a form in her file. I asked her if the doctor could write a letter stating she had a physical done but she said she has since switched providers. Inspection: Renewal. Refrigerator (3270.107)
During the renewal inspection, the temperature was 50° in the Preschool Room refrigerator. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: A new thermometer was bought and placed in the refrigerator. The next day the Director saw that it was 50 degrees. Maintenance was called to look at it. It was determined that there was a large block of ice in the back of it. Classroom staff defrosted the refrigerator and plugged it back in. During the inspection, the smoke detectors were not functional/audible in the Infant Room and Older Toddler Room. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). Correction: DIrector placed a work order with the Lycoming County Housing Authority. They came and tested them and found they were not interconnected. They replaced every single smoke detector in the center and it was still not connecting. They are going to call an Electrician. Inspection: Allocated Unannounced Monitoring. During the inspection, the hot water temperature was 125° F in the Infant Room. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Correction: The Lycoming County Housing Authority came and adjusted the temperature on our hot water heaters. The Director tested the water and it was at 100 degrees. During a weekly review of name-to-face documentation, it was observed and later confirmed with the director that none of the classroom teachers took their children outdoors from 12/9/24 through 12/16/24. Requirement: Weather permitting, children shall be taken out doors daily. Correction: Classrooms will go outside daily weather permitting. Director sent a Bright Wheel message on 12/26/24 to all staff reminding them outside time is required daily. Some of the classrooms also messaged their parents asking them to bring in coats, hats and gloves daily as outside time is required. Director also asked on her Facebook page for donations of hats, gloves, and coats so we have some on site for children that don't have any or forget. Inspection: Unannounced Monitoring. During the inspection, it was verified that Staff #1 and Staff #2 returned to the classroom, with their group of children, but left Child #1 outside alone on the playground for at least three minutes. Child #1 was found walking around on the playground by Staff #3 who returned the child to the classroom. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: All classrooms use lanyards which have the children's names and pictures on them. They also use a whiteboard which indicates who is responsible for which children.
