The fire drill log contained documentation of testing the fire detection system which exceeded 30 days (12/9/2024 to 1/10/2025). Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Emergency info/agreement updated 6 mos (3270.181(c))

Based on documentation, a review and/or update to the emergency contact forms and financial agreements of child #1, #2 and # 3 exceeded 6 months. Requirement: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Child's hands washed (3270.134(a))

Based on observation and discussion with staff #1, the 2 of the 5 children the front classroom did not wash their hands prior to eating lunch. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Name/address/phone release person (3270.124(b)(7))

The emergency contact form of child #1 did not contain the addresses of the release persons designated by parents to whom the child may be released. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Correction: Mom's verbal consent was given to complete the form and the emergency contact address was written in by director. Inspection: Renewal. Information re: special needs (3270.124(b)(5))

The emergency contact form of child #3 did not contain information about the child's special needs as specified by the child's parent, physician, physician's assistant or CRNP, which is needed in an emergency situation or documentation this is not applicable to the child. Requirement: Emergency contact information must include information on the child's special needs, as specified by the child's parent, physician, physician's assistant or CRNP, which is needed in an emergency situation. Toilet and sinks proper height (3270.82(e))

Staff #2 was observed to be holding an able young toddler at the sink to wash their hands. Requirement: Toilets and sinks shall be at proper heights for children using them or shall be easily approached by means of platforms or steps. Building Surfaces (3270.76)

In the downstairs preschool classroom, a road rug play carpet with was curling at the corner and the round alphabet rug was lifting around the edges, both being a tripping hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Emergency plan (3270.27(a)(2))

The emergency plan did not include directions to the location away from the facility in the event of an emergency evacuation. Requirement: The facility shall have an emergency plan that provides for accommodations for evacuation of children from the facility building and evacuation of children to a location away from the facility premises. The evacuation routes and evacuation plans to exit the building may be the same as those required by §3270.94 (relating to fire drills). Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met.
