The health assessment of Child #1, dated 12/2/2025, did not state the child is free from contagious or communicable disease and able in child care. Correction: The parent of Child #1 did not complete the correct form for the Health Assessment. When the parent received the form for the child's physical, it was dated on the day that the form was printed and not the day of the appointment. Inspection: Renewal. Building Surfaces (3270.76)

The corner of the carpet under the Lego table in the older toddler area of the main room was curling, creating a tripping hazard. At the gate between the older and younger toddler spaces, which is accessible to the children, a piece of loose trim was not secured to the wall and exposed 2 sharp pin hails. The corner of the dry-erase board of the marker tray in the main classroom's school-age area is positioned in a high traffic area where children could collide with and injure themselves on the sharp edge. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The carpet in the older toddler room under the Lego table was removed. The trim on the door between the Toddler rooms was fixed that afternoon. A window in the preschool area of the main classroom, which staff said has been opened between seasons when the weather is cool, had a screen with a hole about an inch long and another about 2 inches. In the preschool area's main classroom, the wood panel behind the children's play sink faucet is not securely fastened. Three screws are slightly protruding from the back of the panel, creating a potential scratch or snag risk to children in care. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The screws were tightened on the wood panel of the children's play sink to securely fasten the panel. Inspection: Allocated Unannounced Monitoring. Outside Walkways (3270.64)

The walkways directly outside the exit doors in the in the preschool and older toddler areas of the main classroom were not cleared of snow. Requirement: Outside walkways shall be free from ice, snow, leaves, equipment and other hazards. Correction: The walkway outside of the exit door in the preschool and toddler room were cleared of the snow. Inspection: Allocated Unannounced Monitoring. The file of staff #2 did not contain the correct out of state clearance. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #2 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: Staff #2 obtained the correct out of state clearance immediately and did not need to be removed. The file of Staff #2 contains the correct out of state clearances. The hypothetical location of the fire on the fire drill log was documented as being the furnace for consecutive drills on 7/25/24 and 9/6/2024. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that the hypothetical locations of the fire are rotated around the facility for each drill such that the hypothetical location is never the same for consecutive drills. The fire safety trainings of staff #1 were dated more than 12 months apart (10/13/2023 and 1/3/2025). Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Correction: Staff person #1 completed the Fire Safety training for the following year when she returned from maternity leave. Staff person #1 has a current Fire Safety training. Inspection: Renewal. In the two-year-old area of the main classroom, a child was observed placing a toy animal in their mouth. A staff person told the child to take it out of their mouth, but did not take the toy to be cleaned and sanitized. The child placed the toy back in the toy bin. Requirement: Toys soiled by secretion or excretion shall be cleaned with soap and water, rinsed and sanitized before being used by a child. Correction: The staff person was instructed on the regulations regarding soiled toys. All staff members were reminded that if a child places a toy in their mouth, the soiled toy must be cleaned and sanitized before being used by a child. Inspection: Renewal. Small Toys and Objects (3270.103)

Foam pool noodles were in the discovery table in the older toddler area of the main room. A staff person stated the discovery table was moved to each age group to use, which made the foam pool noodles accessible to children still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: The foam pool noodles in sensory table were removed immediately and thrown away. The sensory table was used by older toddlers and preschoolers while the staff was supervising the children during our summer camp. Inspection: Renewal. The file of staff #1 contained 5.5 hours of annual training in their training year, lacking 6.5 hours. Additional hours were taken after their training year on 6/9/2025 (3 hours), 6/10/2025 (3 hours) and 6/10/2025 (1 hour). Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. Correction: Staff person #1 was on maternity leave during the months that the training hours were not completed. Once the staff person returned, the training hours were completed. The initial health assessment of child #3 was dated 1/9/2025, which was more than 60 days from their start date at the facility (see LIS code sheet). Requirement: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. Correction: The initial health assessment form from 1/9/25 that was past the 60 days, and the current health form is included in the file for child#3.
