Requirement: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first-aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Small Toys and Objects (3280.103)

In the preschool room foam blocks were observed with bite marks in them. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. The file for Child #2 contained an agreement form that was lacking the child's arrival and departure times. Requirement: An agreement shall specify the child's arrival and departure times. All the child care spaces, the playroom, kitchen, and preschool rooms were all lacking a posted emergency transportation plan. Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. The file for Child #4 contained an immunization record that was lacking 1-2 doses of rotavirus, 1 dose of HIB, and 1 dose or pneumococcal. Requirement: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Correction: I will be sure to have the parent of child #4 update the vaccinations. I will notify that parent by 11/16/2025 and give them one week to bring in an updated record. Inspection: Renewal. The file for Child #3 contained a child health report that showed no immunizations and stated that the parents chose not to get the child immunized, however there was no exemption letter on file for the child. Requirement: Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record. Correction: There is an updated health assessment and vaccination record in the file of child #3 As of 11/12/2025 Inspection: Renewal. The fire drill log on file indicated that all of the fire drills were conducted at 10am. Fire drills have not been conducted at different times of the day. Requirement: The primary staff person or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at different times of the day or night, or both, if applicable. Correction: Operator now knows that the fire drills must be at different times each month. Operator will be certain to do this in the future months. Inspection: Unannounced Monitoring. Operator self-reported that Staff Person #1 was responsible for Child #1 when Child #1 left the classroom without Staff Person #1's knowledge. Child #1 was found by the Operator in the upstairs area of the Operator's home hiding in an open closet area. The whereabouts of Child #1 were unknown for approximately 10-15 minutes. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: The worker responsible for Child #1 is no longer working at Frog Hill Daycare. They were let go. Child #1 was found by the Operator in the upstairs area of the Operator's home hiding in an open closet area. The whereabouts of Child #1 were unknown for approximately 10-15 minutes. During this time Child #1 was not physically present with Staff Person #1 who was assigned responsibility for the child. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: The worker responsible for Child #1 was let go. The family of Child #1 found another child care facility and Child #1 was unenrolled at the program in order to stay within legal ratios. Operator self-reported that Staff Person #1 was responsible for Child #1 when Child #1 left the classroom without Staff Person #1's knowledge. Child #1 was found by the Operator in the upstairs area of the Operator's home hiding in an open closet area. The whereabouts of Child #1 were unknown for approximately 10-15 minutes. During this time the child would not have been present with a staff person to be in compliance with the staff:child ratio requirements. Requirement: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 12; Young toddlers 1:5 with a maximum group size of 12; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 12; Young school-age children 1:12 with a maximum group size of 12; Older school-age children 1:15 with a maximum group size of 15. Upon arrival at the facility at 11:05AM there was a lit candle on the kitchen counter within reach of the children. Requirement: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact. Correction: I am no longer using a candle with a wick and real flame. Instead, I use an LED candle for my circle time. Inspection: Allocated Unannounced Monitoring. Building Surfaces (3280.76)

Observed a nail protruding from the floor, which creates a potential hazard, in front of the glass doors in the PSSA Activity Room. Observed two thorny vines growing along the fence in the outdoor play space. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Clean and good repair (3280.102(a))

Observed a broken gas lid, which creates a sharp edge, on a Little Tykes car in the outdoor play space. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. A sticky substance was observed on the floor and shelves of the refrigerator. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: I cleaned the refrigerator on December 6th. The emergency contact form in the file for Child #1, who has been enrolled for more than 6 months, has not been updated by the parent. The emergency contact form and fee agreement in the file for Child #2, who has been enrolled for more than 6 months, have not been updated by the parent. Requirement: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Correction: I have conducted parent/teacher conferences every 6 months and that is when I review the documents in their file, including the emergency contact forms and fee agreements. I write a statement of the review on the parent/teacher conference forms. I failed to sign or have the parents sign a new emergency contact form for review for children 1 and a new emergency contact form and fee agreement for child #2. I corrected this on Dec. 6th. On 11/20/24 at 10:00 AM, Staff Person #1 was observed supervising six children (1 Infant, 1 Young Toddler, 2 Older Toddlers and 2 Preschool Children). Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements at §3280.52(b). Correction: I informed all parents that the new ratio requirement is 4:1 with an infant and 5:1 with a YT until I hire a new worker.
