It was observed in the Teddy Bear classroom that nap mats being used by young toddlers did not have at least 2 feet of space on three sides of the rest equipment. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: All staff were reminded of the proper space required (2 feet of space on 3 sides of a cot), and they adjusted their rest equipment accordingly. Inspection: Renewal. No peeling paint or plaster (3270.77(a))

It was observed in the Teddy Bear classroom damaged paint and exposed plaster on the wall near the windows at the rear of the classroom. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: A work order was put in to fix the damaged paint and exposed plaster on the wall near the windows in the Teddy Bear Classroom. Inspection: Renewal. Building Surfaces (3270.76)

It was observed in the Koala classroom that the diaper changing mat contained tears exposing foam causing a potential choking hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. It was observed in the Koalas classroom a spray bottle containing a cleaning product was not labeled with its contents. It was observed in the Gummy Bear classroom a spray bottle containing a cleaning product was not labeled with its contents. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: The spray bottles were immediately labeled with its contents (bleach and water). Inspection: Renewal. Protective Outlet Covers 5 yrs. or less (3270.65)

It was observed in the Teddy Bear classroom, a surge protector power strip laying on the floor between the refrigerator and a changing table was observed to be missing protective outlet covers. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. 40 square feet wall-to-wall (3270.61(c))

It was observed that the capacity established for classroom 3 (also known as the Panda classroom) was exceeded by two children. The measured space for classroom 3 allows a capacity of 16. On the day of inspection 18 children were present in the child care space. Requirement: Indoor child care space is measured within permanent stationary partitions or walls. The allowable number of children in a space is determined by dividing the total square feet in a space by 40. Correction: The director moved two children from the Panda bear room to a different classroom so that the capacity of the classroom was not exceeded. Inspection: Renewal. Staff person #1 did not complete mandated reporter training within 60 months of the previous training. The current training on file was completed on 9/17/2025 and the previous training was completed on 7/26/2020. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: Staff person #1 has completed the mandated reporter training as of 9/17/2025. The file for staff person #1 included a State Police Clearance, Child Abuse Clearance and FBI clearance which was not updated within 60 months of the previous clearance. The current State Police Clearance is dated 9/17/2025 and the previous was dated 7/21/2020. The current Child Abuse clearance is dated 9/24/2025 and the previous was dated 7/23/2020. The current FBI clearance is dated 9/4/2025 and the previous was dated 8/13/2020. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). It was observed on the toddler playground pieces of mulch accessible to children who are still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: The teachers are taking out brooms daily to the toddler playground to remove mulch, that has migrated from the bigger playground. Any toys or objects that are less then 1 inch will not be accessible to the children as they are still placing objects in their mouths. Inspection: Allocated Unannounced Monitoring. It was observed on the toddler playground a plastic bench used by children is not in good repair by having cracks in many different areas creating rough edges and pinch points that could result in potential injury. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. It was observed in the Panda's classroom multiple books that were broken or torn and not in good condition. The emergency contact form for child #1 does not include the work telephone numbers of the enrolling parent. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Refrigerator (3270.107)

The thermometer in the refrigerator in the lunchroom area indicated a temperature of 50° F. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: A new thermometer was purchased for the refrigerator so that it was showing an accurate temperature reading of the refrigerator. Inspection: Renewal. Handwashing signs (3270.82(h))

There was no handwashing sign posted in the purple bathroom. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.
