During the renewal inspection on 10/09/25, Certification Representative reviewed the file for child #1. The files did not contain the health insurance coverage and policy number for the child. Requirement: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. Written consent/Consent for emergency medical care required prior to admission (3270.124(b)(4)/3270.182(3))

During the renewal inspection on 12/16/2024, Certification Representative reviewed the files for child #1 and child #2. Both files did not contain written consent signed by a parent for emergency medical care. Requirement: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. Consent for administration of minor first-aid required prior to admission (3270.182(5))

During the renewal inspection on 12/16/2024, Certification Representative reviewed the files for child #1 and child #2. Both files did not contain signed parental consent for administration of minor first-aid procedures by facility staff. Requirement: A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission. Restraints prohibited (3270.113(e))

On December 3, 2024, during the complaint investigation, Certification representative observed two children in highchairs that were not eating and two infants in cribs who were awake and not sleeping. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. Correction: The children were immediately removed from the highchairs and cribs to provide free movement. Inspection: Complaints- Legal Location.
