When the QR code was scanned, on the Pediatric CPR/First Aid certificate it showed it belonged to Staff # 1 Requirement: The Department may deny, refuse to renew or revoke a certificate of compliance for fraud or deceit in obtaining or attempting to obtain a certificate of compliance. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -4 may not work in a childcare position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
