During the renewal inspection on 11/19/2024, staff member left two children unattended in the kitchen to answer the door for Certification Representative. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Gave staff member a verbal warning about supervision. Inspection: Renewal. During the renewal inspection on 11/19/2024, Certification Representative observed embedded play equipment in the outdoor play space without protective coverings recommended by the United States Consumer Product Safety Commission. Requirement: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair. Correction: Immediately, we will not use the embedded play equipment. We will research for acceptable protective coverings. During the renewal inspection on 11/19/2024, the facility's exit in the hallway was blocked with furniture. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: We will move the items blocking the exit. During the renewal inspection on 11/19/2024, Certification Representative entered the facility and found only one staff member with two children at the facility. During the renewal inspection on 11/19/2024, Certification Representative observed a staff member exit the classroom which caused the room to be out of ratio. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level).
