At the time of renewal inspection on 10/30/25, the rug in the Reading center of Teetertots and Sunflowers room was stained and in need of cleaning. The rugs in the Reading and Math Manipulative centers in Dynomites room were stained and in need of cleaning. Rug in the Reading corner of Teal Toucans/Sunbeam room was stained and in need of cleaning. Rug in the Reading corner of Purple Penguins/Sunbeams, Orange Owls/Green Parrots, and Dramatic Play rooms were dirty and in need of cleaning. The floor in Dynomites and Orange Owls/Green Parrots rooms were dirty. Some metal nails were protruding from border, which surrounded wood chips on the preschool playground. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. At the time of renewal inspection on 10/30/25 observed that the rest equipment in Teetertots and Sunflowers room was not labeled for the use of a specific child. Mat assignment in Dynomites room was missing. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: Teetertots/Sunflowers have labeled their cots. Dynomites have labeled the cots. Outdoor equip.- protective surfacing (3270.102(c))

At the time of renewal inspection on 10/30/25, it was observed that the depth of wood chips surrounding the outdoor equipment on the preschool playground were insufficiently covering the surface. Requirement: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair. Correction: The wood chips on the playground have been replenished to sufficiently cover playground surface. Inspection: Renewal. At the time of renewal inspection on 10/30/25, chairs in Orange Owls/Green Parrots rooms were dirty. Dolls and stuffed animals in the Dramatic Play area, as well as in the Orange Owls and Green Parrots rooms, were dirty. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Mixed Age Level (3270.52)

At the time of inspection on 6/4/25, staff member 1 was observed entering the Sunflowers room alongside the certification representative and the director. Prior to her entering, there were nine children (ranging in age from older toddlers to preschoolers) in the room with only one staff member present, which is not in compliance with ratio standards. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Two staff must be present with a group of children ranging in age from older toddler to preschool. Correction: The staff was reminded to use the school telephone in the room to call for assistance prior to exiting the classroom for any reason. While this is a split classroom they were also reminded to utilize the staff other side of the room to create an environment with 1 to 6 ratio on each side. Inspection: Unannounced Monitoring. At the time of renewal inspection observed a fire extinguisher located on the floor in the corner of Drama area and hallway. Fire extinguisher was accessible to children. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
