At the time of renewal inspection observed that each classroom and a master file in the office had emergency contact forms that were updated by parents more than 6 months ago. Requirement: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. If emergency information is updated in a master file, it shall be updated accordingly in other facility records. At the time of allocated unannounced inspection on 12/27/24, a health assessment of staff person #1 did not include a physical examination, examination for communicable diseases, and the results of that examination. Requirement: An adult health assessment must include a physical examination. An adult health assessment must include an examination for communicable diseases and the results of that examination. Correction: Staff person #1 contacted her physician to correct the error they made on the health assessment form. The Physician provided a visit summary that included all necessary information for the childcare health assessment. The visit summary is attached to staff person #1's original health form in their staff file. Inspection: Allocated Unannounced Monitoring. At the time of allocated unannounced inspection on 12/27/24, file of staff person #2 did not include verification of experience required for the position of an assistant group supervisor. Requirement: A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Correction: Staff person 32s prior employer was contacted to verify her childcare experience. A letter verifying her experience is now in her staff file. Inspection: Allocated Unannounced Monitoring.
