Requirement: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. If emergency information is updated in a master file, it shall be updated accordingly in other facility records. At the time of allocated unannounced inspection on 12/27/24, a health assessment of staff person #1 did not include a physical examination, examination for communicable diseases, and the results of that examination. Requirement: An adult health assessment must include a physical examination. An adult health assessment must include an examination for communicable diseases and the results of that examination. Correction: Staff person #1 contacted her physician to correct the error they made on the health assessment form. The Physician provided a visit summary that included all necessary information for the childcare health assessment.
