    At time of investigation on 7/16/25, video footage showed and staff person #1 and #2 acknowledged that on 7/11/25 child #1 was left unsupervised on the playground for approximately 5 minutes while the class transitioned inside to the classroom. Staff persons #2 and #3 did not realized the child was missing from the group until child #1 was brought back to the classroom by staff person #1. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. 2. The Legal Entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision/counting of children. The Legal Entity must receive DHS approval of the training content prior to scheduling the training. Certification rep will send a referral to the ELRC to request training. Correction: 1. 7/14/25 - Children will be supervised at all times. Additionally, all staff members will watch a video created by Goddard Systems that goes over the process of threshold counts and name-to-face attendance checks. Staff members have been given a list of reminders about the importance of accurate attendance and supervision. Staff members will use an additional tool to assist in accurate attendance in conjunction with what we already use (iPad with Kaymbu and binder with attendance sheets.) Example - popsicle sticks with children's names. (implemented 8/14/25) 2. 9/1/25 - All staff members will complete a mandatory 2- hour training approved by DHS with a focus on supervision/counting children. The director will wait to be contacted by the ELRC so that a date can be scheduled for the training and all staff will attend. (Implemented 9/12/25) Inspection: Self-Reported Non Compliance.
