At time of renewal inspection on 7/15/25, the health assessment on file for staff person #1 did not have question #2 answered by the physician regarding communicable diseases. Requirement: An adult health assessment must include an examination for communicable diseases and the results of that examination. Correction: Staff person #1 will be notified to return to the doctor's office to have the health assessment form completed in its entirety. Inspection: Renewal. At time of renewal inspection on 7/15/25, a child in the Pre-K A classroom was resting on a cot that was located in front of the emergency exit door. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: The child was immediately moved away from the exit door during the renewal inspection, and was placed in another area of the room Inspection: Renewal. No peeling paint or plaster (3270.77(a))

At time of renewal inspection on 7/15/25, the bathroom located in the Pre-School B classroom had 2 areas of chipped paint on the wall. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Building Surfaces (3270.76)

At time of renewal inspection on 7/15/25, the ceiling vent in the Pre-K A was visibly dirty and in need of cleaning. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. At time of renewal inspection on 7/15/25, the bathroom in the toddler B classroom, which has toilets and sinks counted in the total number, was being used as storage and not available to be used if needed. Requirement: Indoor space in which children are receiving care may not be used simultaneously for other business, commercial, social or another purpose unrelated to the child care being offered. Correction: The bathroom will be cleaned out and remain on the license. The bathroom will be suitable for use if needed and will not be used for storage in the future. Inspection: Renewal. At time of inspection on 11/12/24, staff person #1 acknowledged that on 11/4/24, staff person #2 was holding child #1 while trying to get a bottle out of the bottle warmer. While trying to pull the bottle out of the warmer, condensation on the warmer handle/bottle splashed onto the child's ankle, causing the child to receive a 2nd degree burn from the hot steam/condensation. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Correction: All staff will be trained on the policy of not handling a child while warming or testing bottles. The director will ensure there is no liquid in any classroom that exceeds 110 degrees Fahrenheit. During orientation we will suggest to all infant parents that they refrain from using silicone bottles. Inspection: Self-Reported Non Compliance.
