The last two health assessments on file for Staff Person #1 were dated 1.3.23 and 11.3.25 and the last two health assessments on file for Staff Person #2 were dated 8.24.23 and 11.4.25. This exceeded 24 months between examinations. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. The updates to the fire safety trainings on file for Staff Person's #2, #3, #4 and #5 exceeded 12 months between trainings completed in August 2024 and September 2025. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Emergency Numbers Posted (3270.74)

The telephone numbers of the nearest hospital, police department, fire department, ambulance and poison control center were not posted at the two telephones in the RF1 Classroom. Requirement: The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility. Correction: Emergency phone numbers required were immediately placed above the telephone that was missing the required emergency numbers. Inspection: Renewal. No peeling paint or plaster (3270.77(a))

An area of peeling paint was observed under the paper towel holder in the restroom in the RF1 Classroom. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: The peeling paint was repaired. Handwashing signs (3270.82(h))

Hand washing signs were missing from two toilets in the boy's restroom in the Upstairs Classroom. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Clean, age appropriate (3270.106(a))

One unlabeled pack and play, intended for a child in care, was observed in the LF1 Classroom. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: the unlabeled pack and play was immediately labeled assuring all children were assigned to a crib/pack and play Inspection: Renewal. The emergency transportation plan was not posted in the LB1 Classroom. Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. The files for some of the children in care lacked an area on the emergency contact form for the parents to sign giving permission for walking excursions away from the facility. The facility does participate in walking excursions around the community. Requirement: A child's record shall contain signed parental consent for transportation, walking excursions, swimming and wading. Correction: a permission slip was sent out to all parents to sign giving the center consent to go outside on walks off the premises. Parents signed and returned for all children Inspection: Renewal. The facility lacked verification that they had completed an annual emergency drill within the last 12 months. The most recent emergency drill documentation occurred on 4.2.24. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Correction: The parent was given back the emergency contact to add who shall be contacted in an emergency. Parent listed, parents only in the space. The garbage can in place for used facial tissues in the RF1 Classroom was missing a lid. Used facial tissues were observed in the garbage can. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Correction: Garbage can was immediately removed from childcare space and placed in the office not accessible to children. Inspection: Renewal. The file for Facility Person #2 lacked verification that they had requested their State Police Clearance prior to starting at the facility. A completed State Police Clearance was not on file prior. A completed State Police Clearance was not on file until 133 days after starting at the facility. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
