A multi plug outlet surge protector was observed to be missing two protective receptacle covers in the Infant Classroom. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: Outlet covers were placed on the uncovered outlets. An unlabeled bottle, containing a cleaning solution, was observed in the closet in the Infant Classroom. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: The unlabeled bottle was removed from the infant room Inspection: Renewal. Inaccessible to children (3270.75(b))

A first aid kit was observed on the table in the outdoor play space while preschool age children were playing. The first aid kit was accessible to the children. Requirement: A first-aid kit must be inaccessible to children. Correction: The first aid kit was removed from the accessible area. Building Surfaces (3270.76)

An area of the baseboard heater cover in the Infant Classroom was observed to be detached allowing access to the heating coils. The cover was also observed to be sharp on the edges and at the clip where the cover should connect to the baseboard radiator. Damaged tiles and loose concrete/grout were observed at the entrance to the upstairs restroom. One exposed bolt was observed on the inside of the gate to the outdoor play space. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Stairs, exits, etc. unobstructed (3270.91(a))

The doorway leading from the Infant Classroom to the Toddler Classroom was observed to have a crib mattress across the entry way, a tall basket containing toys laying in front of the mattress and a baby bouncer activity center in front of the basket. This did not allow for the exit to be free and clear. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Prohibit items in crib (3270.106(j))

One of the cribs in the Infant Classroom, labeled for use of a child, was observed to have a bumper pad attached to the inside of the crib. Requirement: Toys, bumper pads, or pillows may not be present in a crib while an infant is sleeping in the crib. Correction: The bumper pad and the crib were removed. The child transitioned to a mat. Staff assigned to specific children (3270.113(a)(1))

At approximately 1:11pm, Staff Person #3 was observed to be seated in the doorway of the Basement B and Basement C classrooms. Staff Person #3 was supervising children who were resting in both Basement B and Basement C classrooms. Basement B and Basement C are two separately measured classrooms, and the requirement is that a staff person is to be physically present in the classroom with the children. A staff person may not supervise children in two separate classrooms. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Services proceeded (3270.123(a)(3))

The child service report on file for Child #2 lacked a date to verify the completion date. Child #2 should have had two assessments on file based on their admission date (refer to LIS code sheet). The file for Child #3 lacked a prior child service report/developmental assessment. The only assessment on file was dated 4.29.26. The file for Child #4 lacked an approved completed developmental assessment and the child has been in care in excess of 6 months. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). The emergency contact forms were not present in the child care spaces/outdoor play spaces where children were observed to be receiving care during the inspection. The facility utilized the Bright Wheel app that included limited, but not all required information, listed on the emergency contact form. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: The director made binders with emergency contact forms in them for all classrooms. They were given to the teachers and they were instructed to utilize them daily. Inspection: Renewal. The written emergency transportation plan was not posted in the Multipurpose Classroom. Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. The emergency contact form for Child #1 lacked signed parental consent for obtaining emergency medical care and administration of minor first aid. Requirement: A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. At the time of the inspection the Staff Files were observed on a desk in the office area. The child files were observed in an unlocked filing cabinet in the office area. The Staff and Child files were stored in a locked cabinet. Requirement: Child records are confidential and shall be stored in a locked cabinet. Facility persons' records are confidential and shall be stored in a locked cabinet. Correction: The files were immediately moved back downstairs to the locked cabinet in the back room. The facility exceeded 30 days between fire alarm testing on 7.3.25 and 8.4.25, 8.4.25 and 9.7.25, 9.7.25 and 10.10.25, 1.4.26 and 2.7.26, and 3.11.26 and 4.14.26. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). Correction: The owner is implementing a plan that requires all staff members to complete one 2 hour training class per month so that they will meet and exceed the required number of training hours. Pertinent Laws & Regulations/General Health and Safety (3270.14/3270.21)

Staff person #3 was observed to be caring for children unsupervised. The file for Staff person #3 lacked verification that they had completed the following pre- service training required prior to caring for children unsupervised: Pediatric First Aid and CPR training in an approved curriculum and the required Health and Safety trainings. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre- service trainings are completed or located, staff person #3 must be supervised, when interacting with children at a minimum by an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #3, staff person #3 may not work in a child-care position at the facility. The facility lacked verification that the emergency plan had been reviewed and updated as needed within the last 12 months. Requirement: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan and kept on file at the facility. The file for Staff Persons #5 and #6 lacked verification that they had received emergency plan training within 90 days of starting at the facility. The files for Staff Persons #1, #2, #3 and #4 lacked verification that they had received emergency plan training within the last 12 months. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. The current Pediatric First Aid and CPR training on file for Staff Person #4 was completed on 11.12.24. The prior certification expired on 10.31.24. This resulted in the training not being renewed on or before the expiration. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. The files for Staff Persons #3 and #4 lacked verification that they had completed annual fire safety training within the last 12 months. The most recent fire safety training on file for Staff Person #3 was dated 2.15.24 and the most recent fire safety training on file for Staff Person #4 was dated 5.8.23. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. If there are no staff person(s) available to supervise staff person #3, staff person #3 may not work in a child-care position at the facility.
