    During the Renewal Inspection on 10/23/2025, the updated health report on file for Child #1, a preschool child was dated 5/5/2025. However, the health report was not signed by a signed by a physician, physician's assistant or a CRNP. The health report for Child #1 also did not include a statement that the child is able to participate in childcare and appears to be free from contagious or communicable disease. Requirement: A health report must be written and signed by a physician, physician's assistant or a CRNP. The signature must include the individual's professional title. A health report shall include a statement that the child is able to participate in childcare and appears to be free from contagious or communicable disease. Correction: I contacted the child's mom, and she was able to get me a note from the doctor that they will do a new physical form that will be signed by the doctor and will include a statement that the child is able to participate in childcare and appears to be free from contagious or communicable disease.     During the Renewal Inspection on 10/23/2025, Staff #1, Staff #2 and Staff #3 all had current mandated reporter training on file dated as follows: Staff #1 8/28/2025, Staff #2 9/2/2025 and Staff #3 9/8/2025. However, all three staff previously went more than 60 months without completing mandated reporter training. This is evidenced by the previously documented mandated reporter training on file being dated as follows: 4/29/2020 for Staff #1, 2/1/2020 for Staff #2 and 5/9/2020 for Staff #3. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). All staff persons must complete mandated reporter training at least once every 60 months.
