During a renewal inspection on 7/6/26, certification rep reviewed the facility's emergency plan. There was no written verification to show a copy of the plan was sent to the local municipality and county emergency management agency. Requirement: The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency. During a renewal inspection on 7/6/26, certification rep observed a plastic truck on the outdoor playground with broken edges which created sharp edges. During a renewal inspection on 7/6/26, certification rep observed several accessible outlets in the indoor play area. During the inspection, children under 5 were using the play are. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Disposable diapers (3270.135(a)(3))

During an allocated unannounced inspection on 3/12/26, certification rep observed an uncovered trash can with soiled diapers in the young toddler classroom. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: A new trash can was purchased and placed in the classroom. Remind staff to make sure garbage can lids are closed at all times. Inspection: Allocated Unannounced Monitoring. During a renewal inspection on 7/23/25, the emergency plan on file did not include a statement regarding the continuity of operations in the event of an emergency. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The director will update the emergency plan by adding a continuity of operations plan. During a renewal inspection on 7/34/25, certification observed more than 30 day between fire alarm test logs. This is evidenced by fire alarm test logs dated 11/20/24 to 1/16/25, 1/16/25 to 2/20/25, and the last fire alarm test log was dated 6/12/25 as of the date of inspection. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. During a renewal inspection on 7/23/25, the legal entity was unable to provide a valid certificate of occupancy. Requirement: A certificate of compliance will not be granted by the Department until the legal entity provides a certificate of occupancy as proof of compliance with the applicable requirements of the Department of Labor and Industry in 34 Pa. Code § 403.23 (relating to child day care facilities) and under section 1016(c) of the act (62 P.S. § 1016(c)). Correction: Church administrator will obtain a certificate of occupancy from Labor and Industry or the City of Scranton. Inspection: Renewal. This is evidenced by the health assessment forms on file dated 6/23/23 and 7/8/25. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Requirement: One first-aid kit per child care group must accompany children and facility persons on excursions from the facility. Each first aid kit taken on an excursion must contain a bottle of water in addition to the items specified at §3270.75(c). Correction: During the inspection, the director gave staff the first aid kit and reminded them that they must bring it outside. Inspection: Renewal.
