   Comply with CPSL/CPSL information (3270.32(a)/3270.192(4))

   During renewal inspection, it was discovered that staff #1 lived outside of the state of PA within the last five years. Out of state clearances were obtained by the director the following day so staff was able to resume working, however out of state clearances were not in the file at the time of inspection. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: Staff #1 had fully completed all required Pennsylvania clearances prior to hire, and her out-of-state clearances were also obtained in accordance with CPSL requirements. Unfortunately, those out-of-state documents were not physically in the staff file on the day of the inspection and were unable to be immediately located due to our administrative specialist being out of the office that day.    Building Surfaces (3270.76)

   During renewal inspection, it was observed in the Pre-K Counts 3 room that a tile was damaged. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: While our building is routinely maintained to high standards of cleanliness and safety, a small crack in the corner of a ceiling tile was noted during inspection in a classroom that was not in use by children at the time. Although the tile did not appear to pose any immediate hazard, we acted out of an abundance of caution and replaced it with a brand-new tile the very same day. Inspection: Renewal.    Adult Hygiene (3270.152)

   During renewal inspection, diaper changing was observed in the toddler room. Although a staff member utilized gloves, the staff did not wash their hands between diaper changes. Requirement: A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child. Correction: The staff member washed her hands immediately following the diaper change, during the process of assisting the child with handwashing at the sink. In addition, the staff member is First Aid certified and was trained in safe glove removal practices to avoid cross-contamination. While gloves are not required by DHS regulations, our center uses them for every diaper change as part of our elevated hygiene standards. Inspection: Renewal.    Health and Safety Training - Pediatric First Aid and CPR (3270.31(f)(10))

   During renewal inspection, it was observed that staff #2 did not obtain a PQAS approved CPR training within 90 days of hire. Requirement: Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Until such time as the required training has been completed, staff person #1- must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. Correction: Staff #2 is a student completing work-study hours through her accredited early childhood education program. Upon placement, she arrived with all required clearances and documentation through her school. While at our center, she was consistently paired with experienced, qualified staff members and was never included in the staff-child ratio. Although she previously completed Pediatric First Aid and CPR training in January 2025 with a certified instructor, that instructor had not yet completed the process to be listed on the state's PQAS-approved provider list. Out of an abundance of caution and commitment to full compliance, Staff #2 successfully repeated the training with a PQAS-approved provider on July 1, 2025. Inspection: Renewal.
